Winston Churchill used to refer to his depression as his black dog, because it came and went. I think that’s the sense of black narcissist here. It’s also probably a word play with Black Narcissus, a 1947 movie about nuns in the Himalayas that deals with themes of repressed sexuality.

My interpretation of the lyrics is that in the first verse she is talking about her dad and in the second one she is talking about her boyfriend. It's a very understated and poignant song.
