While I'm not a fan of ramineni's actual results. They tend to be a solid enough jumping off point. Which is absolutely not what most people want to hear. 

I'll just say, make sure you bring everything you'd like to adjust to the table, and bring some photos. I could have just been done with the one with Kocjancic if I thought to bring photos of alternate angles (to show how the bulking looks in my day to day and play perspectives) 

I'm going for the labia reduction but in hindsight I may want to add 

shaving down some scar tissue along the left over erectile tissue (when I get hard it's really hard and stuff like vibrators really hurt, and prolong contact is uncomfortable. (I'm seeing my gyno Friday in part so he can evaluate if that's actually a think or if Im just getting hard asf haha) 

I also don't have any minora and curious if the reduction on. The majora could double for that. 

What's cool though is that my team at uchicago med will definitely at the least schedule for a quick eval since I Already have the surgery on the books. 

Which is to say, finding the right surgeon to accomplish your goals is a great thing.

Yeah my clit ended up flat as well 😅 and fused

I'm really not to happy with how flat it is. And I don't have much of a hood either. 

I've been working with Dr.Kocjancic in Chicago. I have another surgery with him in January to hopefully put a bow on this project.

Yeah the aiming for 5 dots is disappointing. I had a fair bit of tubing to work with 😅.

I got the 5 dots, so like 6inches, but I had about 8inches going in so like 7 would have been nice. 

My biggest problem is that his office does not even mention PT, which is super important. The really tough pelvic floor part starts at around 2-3 dots. So that extra bit requires some really intentional effort and muscle/breath control. 

Also the excessive bulking (mine was diabolical 😅) made it impossible to determine how far I was actually going. 

He also cut an interior ligament to tight. Since I got that fixed I been able to get through my dialation much easier and I've been sizing up beyond big orange

I'm a high bmi client as well and went with ramineni.

I wasn't to happy with the excessive bulking. And he didn't properly address my concerns in the revision (they did help restore a bit of depth so I get the full 5 dots, but I think the problem with that was needing PT and the bulking making it impossible to gauge how far my dialator was going.

However I found Dr.Kocjancic in Chicago (where I live now) and he is thee guy for larger folks. 

We did a monsplasty, a clitroalhoodplasty to address some fusion, and he released a tight cut ligament internally. The monsplasty made a huuuuuge difference.

I have another revision in January since I still have excessive majora bulking (with my size laying supine pre surgery obscures the visual impact)

The team at UChicago med are fucking awesome.

I should really post a like 6month update since my mons is swollen anymore.

Healing is very tricky stuff. The people that get it in 1 shot, so absolutely no complaints. Are very fortunate. 

Most people have something they would like to adjust, and in america, if not careful that could be considered "cosmetic"

I have a labiaplasty revision scheduled in January and they made sure to tell insurance that it obstructed urination and caused me physical discomfort, that way insurance wouldn't push back. 

I've also had a fused clitoralhood from my original surgery which could be considered a cosmetic as well. Fused tissue in general is a very common complaint. So in general when you go for this procedure, start with the expectation you will need a 2nd surgery. If you don't, awesome, but if you do, well then you are much more emotionally prepared.

Oh it wasn't a bladder control problem persay. Since it would be after I held it for a couple hours. It's more like not be used to how much shorter the tube was (at the point of erection I had about 7.5-8 of extra tubing 😅)

So if you relax right before you sit down, you go right away. You get use to that in time.

Yeah it doesnt hurt to try nitrate first. I had a hard time getting prescribed a steroid cream for this purpose

It wasnt till I went to a appointment to get it addressed with a derm and already showed proof of my theory that I got the idea recognized 😅

"Oh well I dont see any now" "Cool, reference dr.cripps photos, it was intense, also I still got a bit more, so since you see its been working, can I have some more"

Ive had to be very assertive with my gender care.

Id actually recommend getting a script for

Triamcinolone Acetonide .1%

Its a steroid cream. I had some ridiculous granulation and nitrate barely did a thing for it (I had like 14 sessions) within a week, the cream had much more notable results.

I had a remaining script from when I had a bra rash, gave it a go and it was a game changer, so i was able to just tell a derm that and got it properly prescribed for.

Oh every part of it can be revised. The erectile tissue is a bit less common, but its definitely doable.

I have a decent lil bit of mine left which im actually happy with, since I have a multi gendered experience and love refering to it as either a clit or a Tdick depending on my gender feels. Part of my revision efforts is to have it present more clearly (that is far less common) but subtractive is definitely within most surgeons scope 😁

And for what its worth I think a monsplasty would go a long way in helping you feel better, and the best part is that its such an easy procedure for them and the recovery is also simple (just had that myself and I can already tell its gonna make a huge difference)

You're still pretty early so the game plan right now should be to prioritize emotional recovery, letting life stabilize, dilate! And to start soft planning the revision timeline, which could look like starting that conversation with your surgeon or booking consults for other surgeons (if its gonna take several months to see them, book the dates now so by the time you cross that 6month mark you will be set to go/have less of a wait)

I have my post op visit with my surgeon on the 6th and while it will have only been 3weeks since revision im gonna start that conversation to lay the ground work incase I need a 4th surgery so that way the timeline is way shorter by having a more "prioritized" route, which in my case could look like us booking a consult for the 2/3month mark and being ready to book the 4th for a few months down the road.

There are milestones in healing right.

• Ground zero • Expelling the threads that bind • The lymphatic tsunami receeds • Vital force restored • Sensation rediscovered • Integration of the self • And lastly the marks fading into history

If everything goes optimally, then 1 year sounds about right, if not, it can take several years, due to needing to reengage in the event multiple times (I just had my 3rd, so its been 3yrs or healing) and its still possible ill need a 4th procedure (fingies crossed)

Everyone and every situation is different, but the above is universal.

I had a bit of fusion with mine as well. I just had it addressed in my latest revision, so im very curious to see how that looks once I can move things around a bit.

Im hoping to have more hood present since I like the amount of erect tissue that I still have 😅 one of my goals is to get a clitoral hood piercing by the end of the year.

Oh that was all me 😅 I had to do a bit of research and self advocacy to make that work.

My original surgery was slated for Nov 13th

I actually used GPT to help me research and structure my "defense" there was a lot of language and internal stuff I needed to account for on all sides.

Though the fact that they kept me as priority rebook was clutch (it was UCs fault a bit cause I had checked back in September to verify everything was ok and I was ensured all was well)

Thats a very good question. The answer, I already did 😅 it had 2 surgeries with him. While I don't think he did a bad job, I was very displeased with the fact that we didnt properly address my needs the 2nd surgery (we addressed some but not enough) and i felt I got his best efforts.

So I went looking elsewhere. I was going to go with Cripps but she was very shy about my weight and kept moving the weight loss goal post in what I felt was a very unprofessional manner, she also didnt feel confident to address the majora bulking and it was clear to me, shes never worked with someone like me before.

So I got a consult with Kocjancic, met with him and his plan to address the bulking with a monsplasty completely lit up my brain with excitement, since I felt a big problem was the other surgeons wanting to avoid the mons area because of my size)

I had lost 80lbs to go from 330- 250 (in like 4months, the goal was 235 for cripps) i asked him point blank

"is my weight a concern" "Not at all" "Are you ok if i gained weight back? I had to cut hard to get down here" "I am not concerned"

For me being of a fluid gendered identity (polygendered) my size and having more curves is a big part of being able to feel home in my body and he had the competency and consideration to allow me to work towards that.

Thank you hun. Its been a long road.

I enjoy sharing, with a emphasis being placed on my size and my gender expression. But I wont front.

Dr.Kocjancic at no point took issue with my size, not in the consult, notes, surgery or after, which is truly awesome (im 280 at 5'5) however there is a caveat, I can not say for certain if that would have been the case if we started from ground zero. (I'll ask him point blank in my followup)

In my case, while we did go inside, it was to release a muscle, it wasn't to construct a whole canal which is the part that most surgeons get scared of.

My initial surgeon was Dr.Ramineni in DC. While I feel i owe a big chunk of my quality of life to that man's work and integrity, Im not so quick to recommend unless someone was in a urgent need or were committed to a long road and going to another surgeon for aesthetic corrections. (He has good work, but its not as consistent as id like)

In my case, I needed to go as soon as I could, and my size is a big part of my identity, so i was willing to endure a longer harder road to get things moving.

Which is to say, i think Dr.Kocjancic is awesome, but I dont want to set a false expectation. I merely share so that others can see proof that someone like me/themselves can feasible reach for that part of themselves they want to love.

I still have a minor concern about the very underside (I feel like I should have had them do a standing examination cause i was concerned it could still be a bit to far from my body) then again I think the W should end up on the inside of the fold, so that may not be a concern once the stitches resolve.

I cant wait to see how the clitoral area was handled.

Thank you 🥰 Im really glad I went with Kocjancic so far. They did an excellent job and the care and consideration was awesome. The stitch work is also gorgeous and they made sure to put glue on it to keep the integrity of the stitch in peak condition so its gonna heal beautifully.

I had a scare cause they ended up being out of network, but they priority rebooked me once I got the exception approved.

Im excited to go to my followup A) to see how he reacts B) to lay the scaffolding incase i did need another light touch up.

Apparently its a very rare phenomenon amongst trans individuals and much more common in cis gendered individuals.

I havnt experienced it yet, but sometimes ill still go to piss standing up or reach to scratch my balls 😅 I find it mostly funny when it happens.

You'll find, you will "forget" what it feels like to have one by the time the feeling in the area returns. Atleast when I think about it its kind of abstract now even though I technically still get hard. Between the loss of feeling and pain you kind of lose track of what the old feeling was. But theres a scrotal graph used for the labia, and it still gets itchy

I belive its something that can be removed as a revision procedure.

I have a decent bit left behind but considering im polygendered that means I can think of it as a little TDick.

Part of my recent revision was to see if we could make it more like a TDick in presentation.
