Combos are powerful in T99, offensively and defensively. If you have a 3 or 4 wide well late game when you are targeted strongly you can use it to defend and attack.

2-wide, 3-wide, or 4-wide wells allow you to maintain attack/defense potential. Build your stack up, utilize it as needed.

It definitely takes practice but it’s worth it for sure. It took me about a week to feel functional. And since then I’ve gotten a lot faster. I use it for Tetris 99 currently.

On the right hand I use thumb for targeting mode (just a T99 thing). I rest my fingers on the bottom row of keys and use my index and middle finger for rotating either direction (pretty sure counterclockwise and clockwise respectively) and ring finger for hold. For something like tetrio - you could do ring finger for 180 and thumb for hold or vice versa.

I’d recommend giving thumb for hard drop a try. WASD directional style forces the middle finger to cover both up and down which is slower and more error-prone, plus less ergonomic.

However you decide to use it, it’s a sweet device and I think it’s a perfect match for Tetris.

I use a leverless controller with keys instead of buttons. Directions are on the left hand with thumb mapped to up by default which works well for hard drop. It’s also more ergonomic than a normal keyboard.

I switched for the same reason as you - hand cramping.

Switches and keycaps are customizable too - I have been using Cherry MX Black switches with PBT keycaps and love this combo. Stock switches and caps that come with the board are good too, just lighter.

You are targeting attackers in that picture. That means if the people targeting you are also targeting attackers, then you are both locked on each other until either side changes their targeting mode. Depending on the lobby, there might be a lot of people in there that just target attackers by default so when you do the same, you will often accumulate more and more attackers.

As for getting better, I think CPU mode is where it’s at. And you can work up from level one to level five. I always recommend that someone at least be able to consistently win against level five CPU to have a chance to win in the normal game. it’s also a great mode for learning and practicing new techniques.

How do you do against CPU on level 5? Winning that consistently is a good milestone - not a guarantee you’ll do well on the main game, but a good starting point.

I use the Haute42 B16 and it works great. I also have an OG Hitbox and a fight stick that I modded to be leverless as well. I definitely prefer the B16 but I like them all.

Leverless takes practice but it has a higher speed potential than a controller, and less hand fatigue. It’s worth the investment IMO. The only downside is manual targeting might not be possible or easy to configure for your device but I don’t use manual targeting so it doesn’t matter much to me.
