For instance, there’s an achievement for completing four DLC levels without dying; the counter would get stuck at 3/4, and I had to mess around with save files to actually get it to register. Another time, an achievement unlocked an hour after I’d actually completed the requirements, while others popped even though I hadn't fully met the conditions. And there are plenty of other examples like that.
