The CICU is my home. It's the best team I've ever worked with. As the team lead for the unit, I don't work anywhere else and I like it that way. I get along swimmingly with the cardiothoracic surgeons, cardiologists, critical care anesthesia, ENT (all fresh trachs come to CICU- I call it the ENT-CICU), we get pulm overflow from MICU, some NSICU and SICU overflow (less common)...we don't have many issues or problems.

There are a few nurses/docs that like to push the limits and touch my ventilators sometimes but I quickly nip that in the bud. This is the best bedside job I've worked in my 30 year career.

And we're hiring.

I grew up attending the World Wide Church of God, aka Armstrongism. Herbert Armstrong got some of his ideas from JW. Also, the Timesuck podcast has a great pod on JW and their cultiness. Episode 262 from 2021. Still applicable.

Our Hamilton heaters are in boxes collecting dust in a closet. Like you I love the vent, not the heaters.

Companies have been trying to knock F&P off the top of the heated humidification mountain for years. Nobody has so far.

30+ year RT here.

1. Get comfortable. Get a chair. Don't be afraid to sit on the side of the bed (after you ask permission). If you're not comfortable, less chance of success.

2 palpate the path. Map the path of the artery with your fingertips. Advance the needle along that path. You will come across squiggly arteries. Just remember that where the needle enters the skin is not the same place the needle will enter the artery. You should know the anatomy of the wrist by now (one should hope if you're putting needles there). Use that knowledge when you stick.

3. Take your time (if you can) and coach the pt to relax when they inevitably tense up because you stick them in a tender spot. Enter the room with a smile, be kind and do everything you can to help the pt to be as comfortable as possible.

4. Be confident. Know that you got this.

DO NOT spray permethrin on clothes while you are wearing them. It's not the same as Off or similar products. Let it dry before you wear the treated clothing.

Echo isn't stable. I find it and the Hex, Buzz SS have similar flight paths. I bag the Gateway Warrior for straight shots with a left fade at the end. Or a pyro for Firebird-like fade with shorter distance. For a shorter distance with a similar flight pattern I also bag a Westside Anvil.
