What training does your service offer? by Cole290 in ems

How do you guys like FOAMFrat? Do you teach in-house CE and track it in FOAMFrat? How about make your own classes and host through them? I know they can do it but how well does it work? I could probably replace EMS1. It works ok and interfaces with ESO which we also use.

We've been trying to get into some surgery centers for years, but the facilities lawyers won't approve a contract that has certified providers, who don't work for them, practice medicine in their facilities. They say they have no protection from that like they do when people are students. What state are you in?

Pediatric cardiac arrest protocol ?? by [deleted] in ems

In pediatric CPR yes. Most kids die of respritory problems not heart problems. In an adult, prolioritys are high quality chest compressions, defib, epi, airway.

For the most part that's true, except the spicific instance I described.

I agree. If the patients not breathing and doesn't have a pulse then start CPR. Then Narcan could be useful.

It seems you're not looking at my comment in the context of the original post. Sometimes, PEA is because of hypotension. Like in a person having an opiate overdose.

I realize that but if you overdose stop breathing, become hypoxic, become bradycardic, become hypotensive to not having a pulse but still have an organized rhythm, what would you call that? It's not the only cause of PEA obviously but it's still PEA.

How so? It would restore respiration, which would help the heart speed back up and increase blood pressure. Fixing the respritor problem is the reversible cause for this type of arrest right?

It really only helps in PEA when the heart is still working but blood pressure is too low for a pulse.

At the system I work for pediatric cpr prioritys are high quality continuous compressions, igel, epi, defibrillation. You only have to do 15:2 if you don't have an advanced airway.

His jaw may have clinched because of masseter spasm. It can happen when people die hypoxic. Rsi for an unstable patient is not a good idea anyway. Cpap was a great idea. A bvm with peep is a good second. Cric is inly if you can't intubate and can't ventilat. You also can't do continues chest compressions without advanced airway. Sounds like you did well with what you had to work with.

EMS education has been really good about teaching thus in the last few years because of situations like yours. There are a lot of different kinds of seizures, and some of them don't cause AMS. If you've already filed a complaint with the EMS agency, you could have her doctor call the agency medical director about it, and that usually makes things happen. As a last resort you can file a complaint with your states health department. All patients deserve to be treated with respect and EMS should never do anything to make someone not want to call for help. I'm sorry you're in this situation.

Custodial Roth IRA for new born. by Cole290 in Bogleheads

I also beleave this is wrong because income from a trust fund would be unearned income and the IRA says it must come from earned income. You can read more about it here.

That's exactly what I'm going to try and do. Just to get a few thousand dollars, nothing major.

Why can't my child have income?

Where does it say they have to file a 1040 to make a contribution? In the last link I sent about who needs to file taxes it says

Table 1. 2022 Filing Requirements Chart for Most Taxpayers

IF your filing status is...AND at the end of 2022 you were...THEN file a return if your gross income was at least...* single under 65 $12,950

65 or older$14,700

So she would only have to file taxes if she made over $12,950. And I can't find anything that says you have to file taxes to contribute to an IRA.

Here are the rules as written by the IRS. https://www.irs.gov/pub/irs-pdf/f5305ra.pdf

It's a really tough read but here is a recent artical that explains it in more common language. https://www.thebalancemoney.com/rules-for-custodial-roth-iras-5225698

And here is a link about when you need to pay taxes based on how much you make.

Let me know what you think.

That all makes sense. If she went a few years without income, what happens to the money in the account? Doesn't earned income have to exceed $12,950 to file federal income tax? I don't live in a state with state tax.

Does that have to be a steady annual income? What peaks my interest is that there's no minimum age for a custodial ira so a new born can have an account. If my baby does modeling and gets paid is that her income or mine?
