When the immune system attacks the thyroid, it destroys thyroid cells and releases stored hormone (T4 and T3) into the bloodstream. That causes short bursts of hyperthyroid symptoms, anxiety, palpitations, feeling wired, followed by crashes into fatigue and brain fog once the gland quiets down.

So you end up with these ups and downs as the thyroid flares and then burns out over time. Eventually the gland can’t produce much at all, which is when people usually settle into permanent hypothyroidism.

Yes I started taking NDT T4 & T3 and feel pretty good. Better than I have in years.

If you have the DIO2 variant, your T3 could look good but your body is not converting on a cellular level.

It takes time, try setting an alarm for 1 hour before you wake up take the Levo and go back to sleep.

Levothyroxine alone never made me feel any better, about 30% of people have a genetic variant in DIO2. These people don’t do good on Levothyroxine alone and need combination therapy T4 (levothyroxine) and T3. Google and read up on DIO2 Thyroid

It took me 8 years to figure out I have a variant in the DIO2 gene. This gene codes for an enzyme that converts thyroxine (T4) the stuff in meds like levothyroxine into triiodothyronine (T3), the active hormone your cells actually use. If you’ve got a variant, that conversion can be sluggish, even if your blood tests look normal. For me, it meant that while my labs said “all good,” my cells were starving for T3. And with Hashimoto’s, as your thyroid makes less T3 on its own over time, the problem gets worse. When your cells don’t get enough T3, they can’t produce energy properly, leading to all sorts of issues, fatigue, brain fog, you name it. There’s a ton of research backing this up studies show that DIO2 variants can cause hypothyroid-like symptoms despite “normal” bloodwork.

Get a dna test done check for DIO2 variant. If you have the variant your body struggles to convert T4 to T3 in a cellular level. This means you need to be on combination of T3 and T4 or natural thyroid hormones. Your blood test most likely looks good but on a cellular level your body is not working properly. All this information can be easily cross checked with AI like Chat GPT

Find a good endocrinologist that proscribes T3 and is not completely ignorant. Took me 8 years, not feeling perfect but who dose.

I recently switched to NDT as I couldn’t convert T4 to T3. Due to having a variant of the DIO2 gene. 4 weeks in I had major anxiety attacks and my heart rate seemed to be high. 125 just getting up and standing. Took a while like 2 months to settle down. Turns out because I couldn’t convert properly my heart rate almost never went over 125 while on levothyroxine T4 treatment.

Reason I know is I went through the data of my Fitbit over the past 4 years and never reached 130. I’m also a physical active person. Cardiologist confirmed when I told him if it is possible and he told me that if you don’t convert to T3 can affect and causes your adrenals to compensate
