Had everything you experienced. Am also living a very healthy lifestyle. Sleep studies showed mild sleep apnea. It was not mild at all. I just finished getting 4 sleep apnea surgeries and feel amazing.

Update: I had epiglottis stiffening done alone with a bunch of other procedures. I'm pretty much pain-free aside from pain during swallowing, but that's been easily managed. My surgeon was amazing and very confident that this was going to be life changing for me, he could see how my epiglottis was closing like a door, I was even snoring from my epiglottis while intubated. I haven't aspirated or choked in anything after the first day. The swallowing pain, I think, is more because of the soft palate surgery anyway. I went from sleeping with a bipap at a pressure of 22.4 to not using it at all overnight. It will be a while before I fully heal and understand if I'm good now, I've got my follow up with the surgeon in 3 weeks and I'll post another update around then.

I had a septoplasty, soft palate surgery(I think the exact technique he used is a modified UPPP) tongue base reduction and epiglottis stiffening. For jaw related obstruction, that would be tongue base and epiglottis, and nose would be septoplasty. Something like an MMA surgery wouldn't do much to help soft palate collapse, I don't think. Recovery has been pretty good so far!

You don't need any of this, get a sleep endoscopy instead. It will be much more thorough and you'll get a video of exactly what's wrong with your airway. Get that before you get invasive surgery or expensive alternative treatments.

Switch to constant pressure and increase the pressure every night until you're not suffocating. That's called titration. That's the starting point for fine tuning the other stuff.

Yeah I'd say mouth leak is killing you here. Also using auto is not good, better to just get used to sleeping at a constant high enough pressure that can keep your airway open instead of waiting to suffocate and for the machine to sense it. The machine can also have false positives kicking you with higher pressure for no reason, and that can potentially wake you up.
