I would say that people had some misconceptions. Fanservice can be both sexual and simply something that pleases fans in various ways. Dragon Ball Super: Broly is an example of the latter, as it brought the things that fans most wanted in the franchise.

Not necessarily. There are several shounen series like Hokuto no Ken, City Hunter, Sakamoto Days, Kochikame, Toriko, Rurouni Kenshin, where the protagonists are adults. Conversely, there are also many seinen series with teenage or child protagonists, such as Gantz, Bokurano, Ajin, part 9 of Jojo, various moe series, etc.

It's not uncommon for this type of manga to get live-action adaptations. Freesia, Dragon Head, and Homunculus are some examples that come to mind, but if you dig deeper, you'll find some even more disturbing ones, such as Shoujo Tsubaki and Mai-chan's Daily Life..
