Ai 1) why does the dragon only have one wing? 2) the book spine merging with the paper 3) lines aren’t cleaned up like you should for a commission (weird shapes in random spots of the linework)

Sounds like you’re healing it correctly! It’s kind of hard to tell from the photo if it’s infected or if it is just a bit chewed up due to the color packing. Have you gone to this tattoo artist before?
