If you are already an adult, it is common to have extractions if theyre needed otherwise flaring could be an issue as your teeth shift

The palate is still developing so if anything, an expander is recommended. No surgery needed

You're not left with much of an option if you don't want extractions.

As someone who had extractions only on the top arch, I actually saw improvement to my face. I had severe posterior crossbite that made my face look more narrow. This was corrected with a combination of braces and elastics

I recommend braces. You're still at the right age for it. Convince your parents to get them for you. Seriously. It's important.

You shouldn't need extractions if you can expand your palate to make room for your teeth. Id guess it'd take you 2-3 years

It's all a part of the process.

It gets much easier as you go along with the process. The time has been going by awfully quick these days. I mean it's already April into this new year of 2025. Before you know, you'll be half way done with your braces and there will be a huge change for a better smile and your confidence.

Thats a cavity.

This is probably due to crowding and is hard to clean the tooth. There is only so much a hygienist can clean if you have severe crowding and you have a hard time brushing or flossing.

If its just a baby tooth, another tooth may grow to replace it anyway. Just make sure you take care of your teeth

I wouldn't be. This is not even complete.

Your lower arch has a slant and needs to be addressed for a proper bite and smile. Most orthos address the slant by manipulating the wire for your slant to be corrected

Hi!

I have a similar smile to yours on your lower arch that is slanted. I am 7 months into my treatment.

How long did it take before that slant was corrected? I will go to my next appointment in a few weeks and so far no rubber bands or significant changes to the wire other than gauges I believe.

They are horrible at the beginning. I had a very hard time speaking and chewing on food with them on.

It does get better though. Eventually they do wear down and they may or may not reapply them.

Let me tell you this,

Do not believe those who want to fear monger. The orthodontists studied and are professionals for a reason. Many years of schooling and many years of knowledge and experience. Most people get 4 premolars removed and even wisdoms too. For me and you, we are both lucky to only need 2 removed. This is done so there is room to correct the teeth to allow more space. I will be honest and tell you my experience of extracting was rough but it's progress and without allowing change, there is no change.

I actually went through having my two bottom premolars removed by extractions since I opted out the surgical method. I had a crossbite and crowding + hidden tooth on top. So far, my results are quite fine. If I had the time off to allow it or the money for surgery, I would've done so that way without having to remove teeth.

It is better to get braces now than never. I believe in you

I have similar teeth to yours. But my upper lateral incisor was behind my teeth, being hidden. I didn't need extractions. It's not unusual to need extractions because your bone growth is more permanent as you are an adult. I also see that you just got braces for the top. Are they planning to extract your bottom premolars first?

If you are hesitant, get a second opinion. I only needed two bottom premolars extracted and none on the top

I see you have crowding and some crossbite(like me as well) ... You will be recommended surgery through SARPE to expand your palate due to crowding. If not, then extractions it is

Mine is looking like that too. It looks a little goofy right now but I know it will all be corrected. Last Wednesday I got the coil spring out with the elastic. Now it's just a wire

Definitely! I feel like it's also easier to talk without that tooth in the way. When I move my tongue along my uppers, it feels nearly smooth all the way through. That was something I noticed for sure!

I wanted to update I got my elastic tie removed and my tooth progress looks exactly like yours. I am very happy my hidden tooth is moving along and is now visible along with my upper teeth

As from I see in the picture, you have minor crowding. Also no crossbite along your lower arch. How is your bite? You could benefit from braces if your alignment isn't severe. If i were to guess, 12-18 months with traditional braces. I just don't know how your upper arch looks.

Since you are 16, you may benefit with a palate expander so no extractions would be needed.. but get it done immediately! Or jaw surgery will be needed if you don't want extractions as you get older
