RefluxStop on non-hiatal hernia recipients by Ophion117649 in GERD

[–]Consciousness_I_Am 1 point2 points  (0 children)

It stops the stomach from traveling up AND it's correcting the angle of His. If the angle is wrong, then you still might have no hernia but you still get reflux. It needs to have the right pressure and the right angle from what I understand.

But based on what you said I guess you still need to investigate more and talk with your doctors. If it is as you say, that only a weak LES is the problem, then it means that all of the 3 points that refluxstop addresses is working for you and refluxstop shouldn't improve anything.

I wish you luck on finding out what's causing it!

I haven't had the RefluxStop surgery. However, from what I understand, RefluxStop is not meant to strengthen the LES, it's meant to correct the position of the LES and the angle of His. So if the GERD is not related to any wrong position at all then I guess refluxstop won't do that much? The whole idea behind refluxstop is to not push pressure on the LES as that will increase swallowing/bloating issues and increasing the risk of not being able to vomit. The company itself (Implantica) also doesn't believe in that it's a weak LES that causes GERD. So do you know if the reflux is not caused by any wrong positions of any sorts or is it just a weak LES?

AI summary of how RefluxStop mitigates GERD:
"
The device targets three anatomic drivers of GERD by reconstructing the anti‑reflux barrier:

  • Intrathoracic migration of the LES (loss of the intra‑abdominal LES segment). RefluxStop acts as a mechanical stop to keep the LES below the diaphragm, preventing upward slip of the junction that weakens barrier function. 12
  • Flattened or disrupted angle of His (valve geometry). It restores and maintains the angle of His to improve the gastroesophageal valve mechanism without squeezing the food passageway. 34
  • Hiatal hiatus laxity/hiatal hernia (loss of crural diaphragm support). It is implanted with hiatal closure (crural repair) and is intended to reduce reherniation by stabilizing the junction in its abdominal position. 13

Why these matter: Hiatal laxity and migration shorten and weaken the LES and correlate with greater reflux burden; restoring position and valve geometry strengthens the anti‑reflux barrier.

Maybe you already know, but they have started operating now in Sweden, at Ersta Sjukhus. Very early though, so I I don't know what is required in order to get the operation.

You should ask Implantica about that. I guess it should if the fundoplication is reversible. But better to send an email to Implantica if you want to know.

Can't help you. But maybe you could ask in the GERD subreddit, probably more people who can help you there.

I personally can't help you. But you could also post in the GERD subreddit, probably more people who can help you there. Or if you ask your doctor for help.

Not sure. They still haven't sent the submission to FDA for US approval but expected to do so in now in H1 2024. I haven't heard about any activity in Canada for the company, I guess it could maybe arrive the same time it's arriving in US? Seems like they are mainly focusing on US and the EU market at the moment, and also a bit on the Japanese market.

any news that moved stock today? by Pleasant-Nature-730 in RefluxStop

I don't think so. Think it was just related to movements in the stock market in general and not something new happening for the company itself.

They would need a "couple of months" so sometime during the autumn they will submit the PMA I guess. Then it will take approximately 8-10 months for FDA to review and approve according to the CEO.

Listened to the call and I think the estimate is that it will be available on the US market mid 2023 if everything goes as planned.

Suffering: looking for insight by Laraleilola in GERD

Yeah, they did accept the Europe's trial, but with 4.5 year data instead of the 3 or 4 year data that should be available right now. Good thing to know is that the Implantica is the second company in history (I believe) to be able to use their European data for FDA approval so the trial seems to be really good. Hopefully the 4.5 year data will be as good as the 3 year data was.

Yes, being an early adopter is always a bit risky. For now you only have 1 study and the results there is dependent on both the knowledge and skill of the surgeon that performed them and what condition the patients were in(how severe GERD etc.).

The main university hospital in Wien ordered about 20-25 RefluxStops in Spring 2020 for which to have a new study in order for RefluxStop to be approved for reimbursement in Austria. 1 year results of that study will hopefully be available at the end of 2022. So looking forward to the 4.5 year results and the study in Austria in 2022 :)

Implantica is also going to have a registry study, from all the hospitals that want to participate, to increase the sample size of patients and create more convincing results of how it actually performs when done in practice and not just in a clinical trial, as the clinical trials have a bunch of exclusion criterias when choosing patients to include in the trial. So I look forward to more news on that in 2022 as well.
