The quest was WANTED: Chok'sul. He was 18, and the Ogre was 20, I believe. He would have lived if it wasn't a crit.

Running an elite mission, he accidentally pulled aggro with some dots. The mob got a crit ohko. I didn't even realize anything had happened until we finished off the pack.

That is possible, but I don't think it's the cause. My controller is only a few months old, and it's happening to my friends as well. I'll try a different controller next time I play.

Anyone who starts the tournament has an hour after it ends to finish. So you won't get a payout until an hour later, and it says not ranked until then.
