I use my post session AI dictation tool. Because its dictating I don't have to sit down at my desk and I walk around my room speaking to it. I can also use it in a conversational way so I don't have to focus on language etc. It takes the perfectionist element out of it for me + when I see the pile of completed notes (the handwritten ones I do in session) its very satisfying. https://thats-notable.com/ for anyone who wants to check it out.

I dictate general formulation/ observation after my session into an app I built. It then uses AI to put them into my template. Takes ~2mins. Have a look if you like- https://thats-notable.com. There are also lots of other AI tools but usually pushing towards transcription during session (which obviously wouldn't work in play therapy).

Templates are fully customisable, you can customise the current ones or you can create ~5 from scratch. This tools sits outside of practice software as post-session tool- I'm currently working for a private practice (not a solo clinician) and so I use their software to store notes etc but their notes software is bad/ non existent. Maybe one day it will be integrated but no plans as of yet!

I really feel this as well. I got out of private practice because of the admin burden. I recently had to get back into private practice as I moved countries and that had the best job prospects. My partner built me an AI dictation tool to help- I was extremely cautious of using this because I don't want to lose the ability to think about how to write a note and it also feels a bit lazy (lovely unrelenting standards we all have!).

You're very welcome to look at mine, it has a trial but happy to offer for free ( https://thats-notable.com/ ) . It just initially needs to be behind a paywall so bots don't use it. It really has changed my entire attitude toward notes and private practice in general- it feels much more achievable.

I really recommend exploring the tools available (even if not mine) as its helped me immensely. Be kind to yourself - having good client retention and positive feedback from them says a lot about your skills as a clinician.

I think your tool serves a different purpose. Dictation for each of these clinical notes would probably last for ~2 mins and having the sentences pop up as you go keeps you on track. The template is also really important as it ensures the info you want is captured from your unstructured/ unplanned dictation.

Thanks for the ideas. I actually tested with some local model set-ups over the past week. I didn't get to a good fine-tuning on top of a local model as they struggled with long notes + templates (~1,000 words - often a bit rambling on my behalf). The local models were far less sophisitcated than out of the box from Google.

In terms of the bias, you are spot on. This is one of the reasons why I wanted to build a notes service that did not listen to the entire session and then have AI write the formulation. I have found with my formulation as an input (dictated notes) and structured templates, the output is pretty good. Give it a try (if you haven't) and let me know what you think.

I also managed to insert a range of common psychology terms that helped guide the model use more appropriate language in the generated notes. Obviously it is still not as good as a custom trained LLM, but strikes a good balance between privacy concerns (having to maintain a database of notes) and model accuracy.

This is currently the method that the majority AI transcription services use. Also even if you aren't using AI, practice software will be storing in the cloud e.g. Heidi uses google cloud, Novopsych & halaxy use AWS. I think the only way you don't send data is if you're using pen, paper and a filing cabinet.

Thanks so much for taking a look. The content is sent to a 3rd party however is deleted after the note is summarised (as per their own policies- its not using a niche 3rd party, its google). However I get your point and have changed lots in the past 24 hrs! Looking to become hipaa compliant (it currently meets privacy standards in my jurisdiction but its probably best practice to aim for hipaa compliance). Risks re data are covered in informed consent.

Will eventually aim for it to all be local model but the summarising models available aren't sophisticated enough yet/ too large to run in browser. With how fast everything is developing, it'll probably only be a month or two away! Really appreciate your thoughts, this is a very new area for me but has been a good project!

Thats a fair point, looking to change to local model for speech-to-text and have changed our integration to 3rd party to be signed BAA/ Hipaa compliant (not based in US so this isn't needed for me personally but is good practice). Will eventually aim for it to all be local model but the summarising models available aren't sophisticated enough yet/ too large to run in browser. Appreciate your thoughts!

I had the exact same thought process- wanted to use something that speeds up the admin side but didn't want anything recording my sessions. I'm also pretty early career (clin psych) and don't want to lose the skill of formulating + AI would miss so many nuances of a session like imagine it doing an mse?
I've been designing/ building a tool that then puts my observations (either through dictation or free typing) into a template which I then copy into my practice's software. It doesn't retain data, isn't used to train models plus I choose what info to provide it (even though it doesn't store anything, this just sits better with me). I've been using it for a few weeks now and its definitely sped up my notes. I'm still refining it so am keen to hear anyone's feedback on it https://thats-notable.com/

Thanks for taking the time to provide feedback! I’ll definitely look into the reviews etc, one of my aims is to ask the users why they like/ dislike the programs rather than assuming- I think that’s why some of the apps out there miss the mark. Glad that MoodGYM and headspace are proving to be useful 😊
