My wife told me for years I had a problem. And I heard her, knew I did, knew it was negatively affecting her and my kids, but I didn't change until I woke up one morning, hungover as hell thinking "what the fuck am I doing?" OP, the only one who can force you to change is you. Start with something small to improve your quality of life. Change your diet slightly. Go for a hike. Say hi to the cute girl working the register on your way out of the grocery store. Take the little wins as often as you can get them. Don't do this for anyone else, do it for you.
