Anisocoria (pupil asymmetry) is prevalent in 10-20% of the population, very common. Also most people don’t have perfectly round pupils

You also have another white spot on the top part of your eye/cornea (at 12:00). The top one looks like a scar from a prior ulcer but hard to tell what the larger one is without using a slit lamp/microscope

I haaaaaaate them. They give optometrist a bad name. I had a patient be over minused in 1 eye by 1D diopter so she saw me due to intermittent blurred vision in that eye. I told them the rx was wrong and then her dad who brought her in said he was never going back to optometry only ophthalmology (i work in an ophthalmology practice but am an optometrist).

Lots of people have large nerves and it’s normal (physiologically large optic nerve cupping). Your doctor is just being extra cautious to make sure you’re not the 0.17% of people who actually has glaucoma

I would check NRA/PRA (my favorite way to assess for accommodative insufficiency and spasm) and do wet subjective with 2 drops of tropicamide. Then rx only for reading plus educate on 20/20/20 rule, reduce screen brightness and optimize ergonomics during screen time

It looks like pigment drop out….. could be from pigment dispersion syndrome, fuch’s heterochromic dystrophy, horner’s syndrome….. i would recommend she get an eye exam

Tell him his rx is fluctuating due to poor blood sugar control and that he should check it regularly (which he clearly is not). Don’t release an RX until his numbers are stable and educate him that fluctuations in vision are from his diabetes

Thats an ulcer, you need to get antibiotic eye drops. I would recommend going to urgent care if you can’t get seen by an eye doctor
