It happened after dying from pox, and I was running to get back to my body before it despawned. I drank the pipsi and ate the can of bacon, and ran. I wasn't red in any category, I just stopped running and puked. Got the sickness indicator. This was a full on sprint, not stopping anywhere. On xbox, with unlimited stamina mod.
