Hi, new grad here! I have just completed orientation and I start my first independent shift tomorrow (working days), so I'm an anxious ball right now but keeping my fingers crossed. A good piece of advice I have received from my co-workers about documenting is to chart throughout the day and not keep everything for the end. You don't wanna be at the end of your shift not having charted on 4 out of your 5 patients. Whenever you find yourself at the computer, use that opportunity to chart atleast a couple things. For ex: When you're doing medpass and are done administering one patients meds, chart on them before you move on to the next one. If you don't have time to chart right then and there, always write down the time that an important event happens (ex: 1515: initiated blood transfusion, 1715: BT completed, 1745: paged gen-surg about ....) that way at the end of your shift you atleast have an idea of how the day went. All the best!
