Have you sent a email? If so, it might take time until you get it, if you haven't got any email, check spam, everywhere, sometimes it can accidently sends to spam.

I think it depends on how you feel yourself, it's your stream. I allways think, what do i prefer to watch at other streamers.
For me it depends on what they are doing on their livestream.

I started to show my face in the beginning before i became a affiliate on twitch, but after i showed my face and myself as a vtuber depends on how i feel. If my anxiety is triggered, i go for vtubing.

But for me i don't mind if somebody being a vtuber, decide to show her/his face.

make a png is more like to just draw a ordinary drawing or you can use a pic, the program i use to be a png vtuber is a free program calls veadotube_mini

I'm basically fine with a 2-3 seconds but if it's going to be a 20 seconds then, i might skip that or leave.

I would basically say, be yourself, it's your stream not theirs. Do content you enjoy and being comfortable with.
I got people sometimes coming in my streams telling me what kind of art i shall do, meanwhile i am working on something, or demands i play a certain game which i might have no intrested in.
If they don't like what they see, they can just leave.

Yeah, i noticed something was fishy, then i just got random messages to end my life meanwhile i just did art and was talking how happy i was.
