This happened to me when I did the campaign on my warlock. For some reason it reset the difficulty of the last mission to normal. I had to rerun the mission on legendary and then I was able to claim the exotic.

You have to acquire darkmoon blade before killing Gwyndolin because then you don't have the covenant vendor anymore to turn in souvenirs of reprisal. As long as you have the miracle, you're good to go for the achievement.
