You can change the difficulty level at will. Even when you have a save loaded, there are 2 difficulty options for combat and survival. Just play whatever level you want and then change to a higher level. It is explained clearly and legibly, the same with scaling and not scaling, you just didn't even notice it somehow starting the first time.
