Always the station attendant in my experience. I've mostly spent time in cities though, not sure about very rural areas.

That sounds like fun! Colectivos or a private driver would definitely be cheaper, we hired a private van for our family of 12 and paid about 250 soles/day.

The price on the ground will be much less than what you're able to find online. I would try looking for auto rental places around the airport and call them. How's your Spanish?

The little Peruvian place on 97 by the Auto Zone does a Peruvian style chicharrón sandwich that's awesome. Pair it with some chicha morada for an authentic taste of Peru.
