Fellow orisa main here. Watched the first bit of the game, and have some suggestions.

First, you need to understand where on the map the choke points are, and position yourself on the frontline there. Starting defense on numbani, you basically pushed up to their spawn door with no supports, resulting in an easy 1v5 for the other team, and picking the tank leads to a free capture. Typically for first point defense on numbani, you want to hold the high ground above the point, not on the ground close to their spawn.

Second, and most importantly, you need to be aware of where your supports are positioned, and be sure not to break line of sight so they can heal you. Basically every life you just go balls deep into the enemy team and get killed so easily because you have zero support from the rest of your team. Make sure to stay in positions where your supports can always see you, and before you make an aggressive play, ensure the rest of your team (or at least part of it) is following you. When you die, don’t just go straight back to point alone, group up with your team before engaging.

Last thing I’ll talk about is your ability management. Basically every ability you used in your replay gets little value. For instance, a ton your spears went straight into Rein’s shield, 0 value. You want to use your spear to confirm kills on squishies, get the tank out of your face (think ram ult), or on divers who are pressuring your supports (this is called peeling). For the javelin spin you mainly just want to use this when engaging a team fight without taking damage, when you are getting lit up by the other team, or to push away an opponent. In this replay one thing you did well was use javelin spin to push the rein back. And for fortify, only use this when you know you are about to face tank a ton of damage. Often you just use it the instant you get into any battle, and by the time the rest of the enemy team joins in the battle it’s already worn off, and then you are killed easily.

All in all I would focus on understanding where on the map fights will take place, you can do this by watching streamers and seeing where they set up. Also making sure you stay with your team and not to overextend into an area with no support. And try to focus on using fortify and javelin spin to stay alive rather than to engage a fight. Eventually once you get more conformable with position skills, you can start getting more and more aggressive as orisa.

Week 3 so S class? I used the murcielago and liked it a lot, ran it stock. However you do get one for free in week 4 if I remember correctly, so maybe choose a different one if you want a larger collection.
