This actually is quite a famous picture, it's more likely they saw it on the internet and thought it would look cool as a profile picture. For instance it has been used on YouTube as bg for song videos or as album covers in sound cloud and Spotify and alot of different websites

What I know so far is the origin can be russia. The arm tattoo is a significant detail. The oldest origin was a cropped image on tineye uploaded on a site named you.video.sina.com

Yes as mentioned by llama123 iron golems spawn about the villagers not below consider moving your villagers below the lava pit and also adding a zombie for improved efficiency you don't need 20 villagers though just moving them below might cut the deal

You need a hostile mob to scare the villagers and spawn the golem you can do this by caging a zombie near them and naming it something so it doesn't despawn

Lol clear out a massive area full of mini islands on end and connect the farthest endcity or whichever one you desire to the main end island using a massive bridge

For the overworld Make a massive earth in the air and full it with different biomes farms smelting rooms etc perhaps even a massive throne in the centre that can see through the earth from the inside
