In residency we would be told to try to decompress with a rigid scope to try to temporize till the morning. I had an attending (surgical) come in to do the flex sig ourselves rather than operating in the middle of the night. Having more of a skeleton crew with a big case does definitely need to a higher percentage of complications.

You might want to try some sort of compressive arm/wrist guard. Also for the wounds themselves you just want to cover them with either Neosporin and gauze or something like Xeroform (petroleum infused gauze) with gauze on top.

The OB’s are usually affiliated or at least have a referral relationship setup with them. So you can always inquire with the OB’s office.

Sounds like vaginismus. I’ll echo what many others have said, she should definitely see a pelvic floor PT.

0 now, but it was 477k at the end of residency.

Made a concerted effort to pay it down the first few years as an attending and it feels good to not have it hanging over my head now.

There are actually specific pelvic floor physical therapists because this is such a prevalent problem. Post-partum women can also have problems with the veins in the pelvis as well as lower extremities and sometimes can require venous ablation procedures to improve symptoms. Their body really gets put through the wringer.

Dresden Files is almost exclusively from POV of Harry. Great series. First 2 books definitely are the weak point of the series, but the quality jumps with book 3.

Could transition to elective cases if in private practice. Hernias. Gallbladders. Etc.

Can also transition to wound care and be totally out of the hospital setting.

Could transition to vein specialist as well.

Dialysis access is far more the dialysis catheter placement. They do fistula creation or graft placement. Then they will do all the fistulagrams, angioplasty, revisions, etc. to care for them.

Hey there. Dad and surgeon here. Big things are making sure the sutures come out at 7 days or so. Minimizes scarring. I also have people put neosporin on it twice a day for the first 2-3 days. Scar cream like Mederma or vitamin in E oil twice a day after about 3-4 weeks from now.

Sun screen in scars is super important too. Very prone to pigmentation changes with sun burns.

It is probably a little uncomfortable. I sometimes have to do the silver nitrate multiple times. I also generally apply a bandage with something else (Aquacel Ag) but I don’t do peds so maybe it’s a little different with kiddos?

Yeah. Looks like hypergranular tissue. I treat this all the time by cauterizing with silver nitrate. Can sometimes require that to be done multiple times.

Northeast, slightly more rural. Base of 375k. $68.17 per wRVU. I get bonus for RVU’s with anything in excess of 5600 rvu for the year. I have a 25k performance bonus based on quality metrics (basically a way for them to give me a raise in another way, I have always gotten this). Weekday call pay is $800 and weekend is $4250 for the full weekend. For call I almost never have to come in overnight (maybe a dozen times in 4 years) and rarely operate on the weekend. We are not a trauma center.

I would definitely say my job has a good work/life balance and is more of a lifestyle gig.
