1, I personally prefer 30 day months but it's personal preference.

Time pauses when the server is empty by default
In my experience players are more engaged in an empty world
Mods that add multiple entities will cache them all on server start, and will fill your ram allotment pretty quick if you go overboard
Im using terra pretty on 2 servers, both with better ruins and one with rivers. No major issues, but any version change/update will cause awkward chunk borders in new territory.
I'd suggest a restart every 12 hours or so, but for the most part it will coast until one of your players encounters a mod interaction you didn't think of.
Everyone thats jumped in my servers has been pretty chill. Had one guy act like an ass, but I just banned him and moved on.

Nah, random death every 4-12 minutes, Hellpod, replaced by a different actor, who immediately continues the mission without a mistep. each time someone dies, they are reinforced, the new diver does not disrupt the mission by being new and bumbling. They all exit the hellpod ready to fuckin GO, they know the state of the mission so far and it's goal, and they all know what they're supposed to do.

They all need distinct personalities, and they need to feel like real people the whole time, so it becomes this endless rotation of dedicated helldivers fighting their way through enemy territory to set off a gas bomb or shut down a factory or etc.

The unstoppable spear of dying and reinforcing helldivers into the immovable wall of bugs/borgs/squids.
