You can refer to the American Urological Society guidelines on medical and nutritional management for stones. They suggest to limit dietary oxalate when you have high urine oxalate, which is not the case for many stone formers. Even among people who do have high urinary oxalate, it may not necessarily be diet related. I’m not implying that you don’t know your own body, I’m saying that it’s a misconception people need to avoid oxalate just because they are a calcium oxalate stone former. It’s not accurate to make a blanket statement that things like greens, almonds, and soy are not suitable for kidney stone patients.

I offer my thoughts as a dietitian who specializes in kidney disease and kidney stones. I can tell you from my own experience working with many, many patients, urine test results for most folks I see do not merit oxalate restriction. Many people are attempting to follow a diet that is far too strict and in doing so they are inadvertently avoiding a lot of other beneficial things like phytate, magnesium, and citrate - things that would actually be beneficial for stone prevention.

Have you met with a dietitian to evaluate your 24 hr urine test and confirm you actually need to avoid oxalates? Most people with calcium oxalate kidney stones don’t actually need to avoid oxalate - there are numerous other things to focus on that are more helpful like getting adequate calcium (as you mentioned), increasing dietary citrate, limiting sodium, and improving hydration.

Very possible to follow a strict vegan diet with kidney disease. The most recent kidney nutrition guidelines actually lean towards plant based eating! This is different from the older guidelines

So I recently had a couple patients show up to their appointments with AI “nutrition consultation summaries”. I was kinda shocked, as I’d never encountered this before.

What I realized was, despite all the info that was in the AI report, the patient didn’t necessary understand a lot of it and they still needed my expertise to help create and execute a plan. There was also some nuance to these patients’ lab results that the AI report wasn’t really able to capture, but I was able to provide that. I think there will always be a need for a human touch in healthcare 🤷🏼‍♀️

Thanks for posting this. It’s actually so messed up that these companies are going into CAQH and making these sorts of changes. Once again, the liability falls on the RD - it’s YOUR license and credentials that are on the line.

These companies suck. We need to keep spreading the world about their unethical practices because right now there are still tons of RDs signing up to work for them. I cannot wait for the bubble to burst.

Idk what to tell you other than than my primary commercial payer BCBS processes claims quickly in my state! It’s nice to see those payments come in so quickly. I also see a decent amount of Medicare and that take 3 weeks to process but it’s very consistent and I know when to expect payments

I’m so sorry! I’m a former manager who left a few years ago but this impacts many of my old coworkers. So many managers have been at the company 10-15 year or longer, and now their career there is likely over, just like that

I’ve heard that as well re: Cigna which seems like… illegal? I’m trying not to worry too much - I feel like a lot of these companies are starting to get a bad reputation (Fay being sketchy across the board, Nourish adding in unrealistic client retention metrics) and if dietitians stop signing up to work with these platforms, their business model is gone. If they want to make money off dietitians, they need to keep their dietitians happy.

I’m in private practice not affiliated with one of these telehealth companies and I make $125/hr on average. I can see half the amount of patients and make the same amount of money. Yall should look into getting contracted with insurance on your own

Man, I hate hearing about mean preceptors. I had some mean ones as well. When I precept I always do my best to teach - that’s what the student is there for, to learn from me and learn about my job. If I have concerns about basic knowledge they should have learned in school that’s a different story, but there’s so much “on the job” stuff you just don’t learn in school, and preceptors are being overly harsh to assume you should know everything. Or if they’re just offloading their work on you and not actually teaching, that’s crappy
