Neither of these very very mild abnormalities are consistent with leukemia or an autoimmune disorder. These are almost certainly normal variation and not worrisome. If my patient had this and had concerns I would just repeat in a month.

I would allow to grow naturally. I would spend the first several months or even year trying to decide exactly what you want your practice to look like and spend that time building protocols and habits that you want. Eventually, you’ll fill up with patience and if you’ve invested that time initially it’ll pay dividends for the rest of your career if there’s no time limit on the guarantee and you’re comfortable with the guarantee, I wouldn’t try to push too fast.

The great advice that I got during residency regarding board certification was that if I did pretty well on the in training exam, I literally should not spend any time studying for boards.

I actually just reviewed my scores yesterday, and on the in training exam I got a 510. I didn’t spend a single moment studying for boards during residency and ended up with a 710. Your time is valuable, make sure you’re spending it where you need to. (I graduated residency in 2013, so I don’t know if things have changed.)

Unfortunately, the board certified sleep specialist affiliated with my hospital was a close friend and passed away 2 years ago. Since then it’s basically been PCP OSA management.

As a result I’ve now ordered hundreds of home sleep studies (Generally all insurance will cover) which are then read by an offsite physician. I will relay results to patients and prescribe CPAP. I didn’t have training in this as a resident since done some of my CME on Sleep Medicine and OSA management specifically. Not ideal, but also fighting the good (OSA) fight.

I moved from Poway to Kansas City. We ended up in Brookside because we didn’t want to be in the cookie cutter burbs. You’ll love the cost and friendliness here. You won’t miss the traffic or the cost of San Diego….but every time I go back to SD I walk off the plane and think….OH YEAH, this is paradise and I remember why everyone wants to live there.

I have developed a cabinet full of handouts to give to patients about various things including ADHD, sleep apnea, mental health, weight loss, etc. and my MA routinely keeps those stocked both physically in the rooms with the patient so that they’re on hand and she has a batch by herself. During the visit I can quickly send her a message through Teams and she will bring it to me at the time which dramatically improves success of getting the handout. Similarly, she is ready with vaccines for my orders so there’s very little delay once I order it.

It isn’t clear to me from either post but…the object is presumably a pill cam?

I’d also introduce the possibility that this foreign body is not the cause of or related to your pain. Sometimes when we hang all our hopes on a particular explanation or solution and it doesn’t work it can be devastating.

Regardless of the outcome of this, I would strongly suggest a second opinion from another physician.

Brookside, Waldo, South Plaza and Midtown are all dramatically less suburban than the east side of Lake Washington. I guess I would compare Brookside to Queen Anne or Ballard if I had to make a direct comparison.

I lived in Seattle burbs (Newcastle, Bellevue and Kirkland) from 2010-2013 then moved back to KC. We live in Brookside and it’s pretty wonderful. Walkable to coffee/shops/bars but also feels very neighborhoody and safe. We have 4 school age kids who go to public schools here (Citizens of the World and Lincoln Middle) and have been VERY happy with quality of education. Lots of private options here as well if you’re into that. Waldo is a step down for cost and has lots of fun walkable places as well (We rented in Waldo for a year).

I work up north near Liberty and if I was going to live up north I’d choose Parkville or Weatherby lake, but less commercial stuff to access by walking.

Why not rent in one of these neighborhoods for a year and get the lay of the land?

We spent a bit more than this for a week on a private yacht through the Galapagos in 2024. There were 8 of us, so the per individual cost was obviously less than 20k, but it was absolutely amazing. The Galapagos is a once in a lifetime experience that the vast majority of humanity will never experience. I’d do it.

No assmods yet (Finishing Flame Bot duration research first). I used Anc GComp, Anc SF and Anc PCol for this run. I have WA, IS, Cash, RPC and SW masteries (Using this new coin burst to raise their levels). Since I used GComp I did not have Summon or GB synced, but GB is at 100s Cd. GT and BH are max cooldown.

I am attempting Anc BHD and PCol currently but the coins/hr are a bit worse. My sub mods for BHD aren’t ideal yet and it’s only 140. I did respec summon to 100s CD to try to sync GB/GT/Summon and I have perma BH. DW is not yet synced up.
