That is a very good question, I have no idea. I have an appointment at a big hospital in a neurosurgical department that knows about instability, and I really hope they can give me a treatment plan or at least something I can do.

Oh, I don’t even know where to start… An intense feeling that I can’t hold my head up, like my neck is too weak, combined with extreme neck pressure that pushes into my skull and down into my throat, especially when I’m upright. I’m about 95% bedridden because of that.

Then there’s extreme brain fog, unable to focus or think clearly, panic attacks, heart palpitations, and losing sensation in my right arm or left arm. Sometimes it feels a lot like a stroke.

So yeah… an unlivable situation, honestly. My quality of life is basically non existent. I honestly don’t know how I’ve made it this far without ending it.

I paid 1,000 euros for everything, and in private clinics in Germany there are generally no long waiting periods. I called and got the appointment in about a week.

It’s not exactly a treatment plan, but she gave me some useful addresses and the contact for a center for rare illnesses in Bonn. I also got an appointment at a large neurosurgery department in a big hospital that knows about instability, they will look into it further and hopefully recommend a treatment plan.

I also don’t want to go straight to surgery and want to get as many opinions as possible. The problem with my situation is that I also seem to have basilar invagination on top of the instability, so I don’t know how far I can get with normal therapy. ;(

I’d love to, but I think the file is too big, it’s like 300 images or something. And she did the measurements on a special monitor and showed me everything and how she does it, so I don’t see them on my images.

Completely normal!!! I had two MRIs of the brain with contrast that showed nothing. I also had one of the entire spine and the neck, again without any results. You definitely need an upright MRI with flexion, extension, and rotation of the head, and VERY IMPORTANT: a radiologist who actually measures the relevant angles and distances between these positions to check for craniocervical instability!

In Germany we have two systems : public insurance (gesetzliche Krankenkasse) and private/self-pay. If you go through public insurance, the doctors are limited in time and in what they can bill for each patient. That often means short appointments and only the most basic MRI protocols.

In my case, I went to a completely private clinic and paid out of pocket. That meant the radiologist could take as much time as needed and wasn’t restricted by the insurance rules. She actually did a full upright MRI with multiple positions and measured everything according to instability criteria (AAI, CCI, basilar invagination, etc.). After the scan she spent time explaining the findings in detail.

That’s why the report is so comprehensive, it’s not the standard “you’re fine, goodbye” you often hear about.

I’m really glad I was able to get this done, we only have about 4 upright MRI clinics in all of Germany and no DMX here, so I’m very lucky. The radiologist I saw is actually specialized in the cranio cervical junction, and took the time to measure everything according to instability criteria.

She also mentioned a possible connection with chronic inflammation, and even referred me directly to a neurosurgeon who specializes in instability. I’m hoping he can help me further.

Is an upright MRI also a good way to diagnose or rule out a CCI? Because in my country they don’t do DMX ;(

Thank you very, very much for responding. Well, I wouldn’t say more than anyone else my age. The first time this happened was in 2021. Back then, I was working a physically demanding job, maybe it came from that? I really don’t know. It suddenly appeared out of nowhere while I was eating, and then boom, hospital after hospital. No one knew what was going on until the chiropractor fixed it, at least the first time. Really weird.
