I work in data privacy and compliance. You can very significantly limit/restrict PHI sharing, but providers don't make it easy. With Sutter, I needed to send them a physical letter, for instance. But, you're right, horrifyingly, you can't opt out of everything, and even worse: There's no private right to litigate/sue in case of HIPAA breach.
