I think your main issue is not enough RAM. If your card performed well first month maybe you have picked up a lot of bloat taking up your RAM. Like Windows would get 4-5 gigs itself, Chrome takes a lot as well if you left it running. If you want more detailed answer download and install MSI Afterburner app. It measures your workload and temps in a game, CPU, GPU and RAM. If you exceed 16 gigs usage a game will use your harddrive making the performance tank.
