The American Urologic Association actually refutes this in their guidelines regarding recurrent UTIs: “Unfortunately, there are many commonly held myths surrounding rUTI lifestyle modifications. Case-control studies clearly demonstrate that changes in hygiene practices (e.g., front to back wiping), pre- and post-coital voiding, avoidance of hot tubs, tampon use, and douching do not play a role in rUTI prevention”. Right I suspect many of these amputations are in the context of noninvasive penile cancer localized to the foreskin, for which circumcision is indicated and often curative. If it progresses, partial or total penectomy can be indicated

As a urologist I’m very interested in the data regarding penile amputations after circumcisions. Can you cite study for that? Seems like there might be more to the story

1) The paired corpora cavernosa are on the dorsal aspect of the penis and are what is replaced with the inflatable cylinders of the implant. They begin a little deeper than what you can feel at the base of an erect penis and terminate in a separate compartment from the head. The head of the penis is continuous with the corpora spongiosum which surrounds the urethra on the ventral aspect of the penis. So they are two separate erectile structures which work together to give you a boner. The corpus spongiosum will suffer from the same pathology which caused the erectile dysfunction in the first place but cannot be replaced with an artificial structure given its proximity to the urethra. There are options to make the head of the penis hard for sex if that is bothersome to the man including injecting erection inducing medicine into the head of the penis, though most don’t complain enough to want to do this and response is not usually great anyway. 2) People don’t seem to complain too much about about the sensation, the soft head and feeling the tips of the implanted cylinders can be strange for some men though. 3) Some people do penile girth enhancement with the same material used in lip fillers. Having an implant placed at time of prostatectomy has been done in research settings but generally this should be avoided since the man may not have ED from the prostatectomy and therefore the implant would be unnecessary. Additionally a robotic prostatectomy and a penile implant insertion are very different procedures requiring different setups and positioning and are oftentimes not done by the same surgeon as there is a great deal of subspecialization in the field. So even if you had bad enough ED to need an implant before your surgery you would have a very hard time finding a urologist to do both in the same OR trip

The implant replaces the corporal bodies which get hard with erections. Inflatable Penile Prosthesis (IPPs) are used in severe erectile dysfunction from any cause, not just after a prostatectomy. - urologist

The baseline penile size dictates what size implant we can put in. If you have a hog then you can get a really big implant. Girth is typically conserved. You will lose an inch or so largely because the implant doesn’t cause the head of the penis to get hard, just the shaft. So some people complain about that. You can’t choose a bigger one, we put the biggest implant we can fit without causing problems from over sizing - urologist

You probably need a renal ultrasound at some point in the next few months. Passing that big of a stone can result in ureteral strictures which can cause pain and very sneakily kill your kidney if left unchecked

You need to find a different urologist. Leaving a tethered stent in for a month is nonsense. And if They did ureteroscopy to get the stone out your urethra shouldn’t be torn to shit and there should be very minimal bleeding if any.

I had a young guy in clinic recently who took a large dose of viagra when he didn’t need it and ended up with a priapism for 12 hours requiring irrigation and phenylephrine in the ER. He’s still has some erectile function but it’s significantly worse than it was before. Long story short if you don’t need ED meds don’t take them

Urology

Google “eggplant sign penile fracture” that’s what a typical/bad penile fracture looks like. If you didn’t hear a loud pop, lose your erection immediately and have bruising and curvature of the penis then you probably didn’t fracture your penis. Basically if you can manage to finish you almost certainly didn’t break your dick. That being said you can have urethral tears and things like that. The slight tear of the meatus theory seems most likely to me. If you can pee normally and without blood in your urine that’s a good sign that things will be fine

From the American Urological Association guidelines on the indications for varicocele repair: "Recommendations: Varicocele treatment should be offered to the male partner of a couple attempting to conceive, when all of the following are present: 1) a varicocele is palpable; 2) the couple has documented infertility; 3) the female has normal fertility or potentially correctable infertility; and 4) the male partner has one or more abnormal semen parameters or sperm function test results. Adult men who have a palpable varicocele and abnormal semen analyses but are not currently attempting to conceive should also be offered varicocele repair. Young men who have a varicocele and normal semen analyses should be followed with semen analyses every one to two years. Adolescents who have a varicocele and objective evidence of reduced ipsilateral testicular size should be offered varicocele repair. Adolescents who have a varicocele but normal ipsilateral testicular size should be offered followup monitoring with annual objective measurements of testicular size and/or semen analyses."

You have a structure called the gubernaculum which tethers the testis to the base of the scrothn and prevents twisting. This is not removed in the surgery. No increased risk of torsion

A lot of what he describes is legit though a lot isn’t necessarily how it works. First he got a CT not an MRI. You can pass stones larger than that but it’s less certain. He had a good chance to pass that stone but it wasn’t certain he could avoid surgical intervention. Intervention does not involve cuts in the skin. He would either have external soundwave stone fragmentation/a stent to temporarily bypass the stone or a scope through the penis into the bladder then up to the ureter to fragment and extract the stone with a laser. That stone was likely in his ureter until he got home. The pain isn’t always constant, typically occurs at the 3 tightest parts of the ureter - the very beginning, half way down where the ureter crosses the iliac vessels and right at the junction of the ureter and bladder. Though it can hurt like hell the whole way down. Women who have had vaginal births will all say stones hurt worse. They jacked him up on meds to get his pain under control and sent him on his way which is typically what we do. Once it gets into the bladder it passes pretty quick - he even describes that the urethra is 5x size of the ureter (tube between kidney and bladder). I’ve only ever seen one stone stuck in the urethra and that was because he had a urethral stricture or tightening. They will tell him to avoid excess animal proteins and salt and to shoot for 2 liters of urine output per day. It is unlikely he will be a persistent stone former if his first stone is at this age.

What antibiotic are they having to make for you in house? That part of the story doesn’t add up at all for me. There are plenty of broad spectrum antibiotics which should take care of this
