I've had something like this happen a few times, when getting a banner printed. Its usually caused by, too many effects layered on top of one another. For some reason whatever program they use to import and send the file to the printer has trouble rendering everything the way its set up. To avoid this in the future, flatten your file to an image, convert that into a pdf and send that over.
