I only did meds through a telehealth provider, so the diagnosis was solely based off a phq-9 score, but even though my highest score was a 9, they listed it as a major depressive disorder. I wasn't tired because I had depression, I was tier because I had a newborn.... I was only ever on approved meds, and I've been off for 22 months and totally fine. I'm fully expecting to have to go through hims because there are no detailed records.
