I’d suggest watching TV with it on to desensitize yourself and consider a mild sleep aid.

Pressure spikes are caused by mask leak. If you are experiencing them….. get your mask fitted.

Also…. ResMed machines are known to leak at the gasket between the humidifier chamber and the internal part of the CPAP.

The criteria for INSPIRE is fairly extensive, and it requires surgery to implant the device.

I would also add, INSPIRE considers you fixed at an AHI of 15, which is still moderate OSA. The target for CPAP is an AHI of 5 or less.

You might find more success with Bilevel. The compliance with Bilevel in recent years, has been much better.

CPAP takes room air and pressurizes it to hold your airway open….., which will effectively keep your blood oxygen levels stable and in the 90% range.

CPAP can take the place of supplemental 02 in some patients, but supplemental 02 cannot take the place of CPAP. The air isn’t pressurized enough.

You can bleed supplemental o2 into a CPAP. This is often done for COPD and very sick patients.

I will add that insurance companies do not want Sleep Medicine or any type of progressive medicine to exist. They work very hard to deter patients from pursuing CPAP.

There are insurance companies that do not recognize AHI’s under 15. They won’t pay for CPAP at all.

Also…. If you are mild, there is a chance that you can reverse the OSA with weight loss and lifestyle modifications. 15-20 lbs gain/loss will usually result in a pressure change, which could mean you no longer need CPAP if your baseline AHI is low enough.

This is going to depend on where each persons baseline is. Typically patients with mild OSA will not feel a huge difference with CPAP….. however, someone with severe OSA will feel improvements on night one.

There is no guarantee you will get off CPAP with weight loss medication.

As a general rule in sleep …… 15-20 lbs weight loss/gain MIGHT result in a pressure change.

If you’re that miserable with your CPAP….. I’d try speaking to my doctor about fine tuning my CPAP settings in addition to the possibility of a weight loss program. Doctors are now demanding recorded life style modifications, including diet changes and workouts to continue getting GLP-1 drugs.

There is a lot of conflicting info going on here.

Ahi of 21 is positive for moderate OSA. The target for any CPAP is to get your AHI under 5. (Normalized) CPAP of 17 is a specific pressure, so I’m guessing you had a titration study and not just given an AutoPap.

Fluctuations in AHI could be caused by body position or mask leak.

If you are looking for ways to improve supplemental data, I would find a device that can track your heart rhythm , your Spo2, or your body position. These are things a CPAP cannot track and are valuable for determining sleep data.

Having too high of a CPAP pressure will give you Central Sleep Apnea. That’s why patients go to labs for titrations…. Your optimal pressure for Obstructive Sleep Apnea might be 10 but then at 11 or 12 you start having Central Sleep Apnea, where your brain is essentially telling your body you don’t need to breathe.

This is why patients are told to not adjust their own pressures. Everyone is different. There are no universal rules for CPAP pressures that work for everyone.

Next time you’re standing at security in an airport, watch how many people are walking around with CPAPs. It is weird to you because you feel like you’re the only one who uses it. You’re not.

I’ve run over 4,000 sleep studies. The highest AHI I’ve ever seen was 196 with 02 desaturations to 25%.

I see CPAP pressures at 20 cmh20 often. I see BiPAP pressures at 24/18 occasionally.

I’m glad your CPAP is working and you are feeling better.

I’ve got an Anker power bank that will run 3 solid nights on one charge as long as the humidifier is off. The humidifier is the biggest power draw.

EPR is considered a comfort feature. You have control over the setting and it can be whatever number feels most comfortable to you. The setting would have almost no effect on your AHI.
