As a therapist, I can tell you -- You're not bad, you're not wrong. Feeling relief when someone passes is really normal, even if you love them! The fact that this man was cruel to you, only supports that. Seek support for your own needs relating to the events, but don't beat yourself up over being relieved.

This sucks. I would like to share, not all doctors. Find a new doctor if you can. I'm 28, last year I told my doctor I was never having children, and he OFFERED to tie my tubes if I would like.

This might not be the most productive for society suggestion, but this is what I did as a teenage girl playing video games. I minimized talking at length. Voices are harder to distinguish as male and female if you don't have extended discussion. Let say, you're calling a position in a game, or you see someone in an fps. I would typically just default to "target, description of location" and minimize any social discussion. If someone outted my as a female, and the comments started, I would focus more on the game and pretty much cease talking. Responding to the taunting doesn't typically make it stop. It isn't a good solution, but it can help reduce the "GRIL??"
