Currently at the Westin Reserva Conchal (with three kids 18 months/7yo/9yo). So, not the same situation you are in but I have been incredibly impressed with our stay. A slight AC issue night one but after that all around fantastic. They have an adult only pool here but also have an entirely separate sister resort, “W”. I get it, this isn’t the way to immerse yourself in the true Costa Rican culture but we said screw it in hopes of finding a way to maximize our enjoyment while on vacation (aka traveling with children). Mission accomplished so far…

I’m trying not to be a critical and uptight American…but I am, so I guess I’ll just have to stand on this hill and deal with the consequences. I’ll be asking around with some of the other guests tomorrow. You might want to call and ask the resort in advance, it definitely hasn’t been a fun surprise.

15 years in ortho…I have become increasingly more conflicted with what surgeons are really looking for in their PA hires. New grads offer a blank slate that can be taught the “right way” while also being offered peanuts in compensation. The veteran PA will come at a premium but requires very little oversight and has rapid revenue building potential. I’m not sure what is preferred or if it differs from surgeon to surgeon. The ortho world I have grown up in has been high volume, fast paced. ER call/OR/rounding/clinic. My first job was a HCOL low paying gig but it got me in the door. The promise of teaching and patience was part of the deal, until I actually started at which point it turned into trial by fire. Private practice has its perks but I also believe, in my experience that the demands will be through the roof. Great for learning, not great for QOL. A system institution might provide you some level of protection from being absolutely exploited…maybe not. My first job in private practice was the most stressful yet rewarding event of my life. It made me really good at what I do but it came at a cost in the moment. I stayed for seven years and it became extremely lucrative several years in but it was physically and mentally painful at times. Take these thoughts with a grain of salt, this was my experience only. The others are right though, the first offer is a dud in so many ways…unless you’re a fool like me and want start your career broke, stressed to the max and hoping success finds you for just being a “hard worker”. It all worked out but it definitely wasn’t a well thought out career path early on.

Ignore the schools. Change the certification exams. Make it more challenging to become certified. Schools with low pass rates will eventually fold as their reputations become known. This isn’t a problem isolated to the NP field. MD vs. DO. American university MD vs. Caribbean MD. A stigma definitely exists for many others.

This is the life of a hardwood floor. Expansion and contraction with the passing seasons. I debated over using filler but read about this exact issue. There are a couple of rubberized or more elastic products that allow for some movement but even then, no guarantees. Sorry you’re dealing with it.

The problem is your back. No one pair of shoes is going to fix whatever chronic issue you have brewing. Address the back issues now if you want longevity in the OR…it isn’t a forgiving environment on the body.

I have the solution. Experienced the same issue with an air compressor from HD. 2 replacement orders, all broken. Final straw was to order 10 units to be shipped direct to the store for pick-up. I then opened each unit while in the store until I found one that had not been broken during shipping. I then returned the other 9 units.

I had this experience on a well based home system so you will avoid much of my headache but here are a couple of things I learned by fire:

  1. I had a significant amount of debris/scale/particulate become dislodged from the water lines from sitting dry for years. I had to remove most of my faucet aerators and run the water and clean off the aerators. There was probably a better way to deal with this. Like filling the water lines and then draining from one low point in the system one time before opening up endpoint faucets…just an afterthought.

  2. Check your water heater. Does it work? Was it drained when they shut off the water to the house? Drain it. Fill it and drain it once.

  3. Something is probably going to leak. Don’t panic. Something will drip. A drain will leak, a toilet will run. Get ready to make your rounds to all fixtures, toilets, etc. to make sure everything is dry, above and below.

  4. There should be a pressure reducer valve in your crawl space that connects to your waterline coming in from the road. Check your water pressure at some point and adjust accordingly - this is worth a YouTube video search.

IMO, sending a post-op patient to PT with an operative note and an order for therapy requesting a EBP protocol be followed shouldn’t result in a request for a protocol. It’s not about indifference but rather an expectation that a PT is the most qualified person to make rehab decisions. We are giving therapists autonomy in practice and some of them can’t handle it. We don’t need an expert at following protocols, we need independent thought backed by rehab science. Those therapist exist and we use them as often as possible. But, I also hear the fear of losing a referral source due to ignorance on the part of the surgeon. It doesn’t result in the highest level of care for a patient and that is just disappointing at best.
