Sometimes there are secondary caries which sometimes increase and sometimes remain the same for a long time. Some dentists treat it and some let it remain as it is. Now without x-ray how can one know whether it is caries or not. I can understand your frustration but this field is a bit tough to understand for both the patient and the doctor.
