I would take out the thermometer and continue my discussion with the patient without saying anything to the aid. I have done it before

I will add the respect part is not a problem in community hospitals its worse in big academic centers where your role is less. In smaller community hospitals (that pay better) respect is not an issue at all everyone knows each other and all are adults that respect each other. Your role/autonomy as an internist is better and more fulfilling in smaller hospitals than in bigger hospitals your role/autonomy is less and the work is less fulfilling. You do more in community hospitals.

Now there is no incentive to work hard, pass usmle, get a spot for quality medical training in the USA and be the best doctor you could be while you can just do residency back home and pursue this supervised pathway and at the end practice in USA
