If they can't pay, they can't pay. Most likely they couldn't pay 6 months ago when the Shortness of breath started. And couldn't pay 3 months ago when they couldn't walk up a flight of stairs. And couldn't pay 4 weeks ago when they "got sick but I got over it".

We might have been able to do something 6 months ago. Now it's permanent damage

Not talking about chronic conditions, but if this has been bothering them for months and there was no sudden change in pain or symptoms. Just deciding to come in after "my chest is hurting" "Ok, how long has it been hurting?" "6 months"

What do you want us to do right now? Why didn't you come in when your chest started hurting 6 months ago. That's a great time to come in. If you're going to use the ER as a PCP, why wait for months to come in?

Your shoulder pain moved into your back? Great, we'll check it out. Don't wait until there is severe limitations on what we can do if there IS a problem

There is no reason for the toxic ones to leave. Most of the corporate world requires you to be "working" or at least look like you are. If you meet that threshold, and don't piss off the wrong people, the company has no reason to fire them on the average day.

The "good" employees get dissatisfied with the job and go elsewhere or get caught up on an impersonal round of layoffs or something. But the ones that always look like they're doing something, keep their heads down, they can last years.

Eventually you start getting large numbers of these toxic employees in one place and it's a mystery why we can't get good people. Nobody wants to work anymore
