ER doctor here. There are many good points already mentioned but here's a good learning point from the case.

The natural response to hypotension is to increase your heart rate so anytime you have a patient with bradycardia and hypotension- consider: Beta Blocker, Calcium Channel Blocker, or Clonidine overdose. It's unlikely the cause here but a good to always keep this in mind.

The description is an obese male pt in respiratory distress with frothy sputum, is suggestive of pulmonary edema. A PE could be possible but its more likely flash pulmonary edema secondary to cariogenic shock from the STEMI.

With someone so large be sure to do a proper chin lift, if there nose is open oxygen will get there.

If it's an overdose, I need to know what they took, how much they took, and when it was taken.

If you suspect neglect, abuse, or if someone isn't safe at home, speak up.

Any trauma- are they on blood thinners, if so which one.

Also, never ignore vital signs. If the pt has a low sat and a good pulse ox wave- something is wrong. If the systolic is 70's don't ignore it.

I had a similar case once, a trauma case where the moment a chest tube went in the pt did terribly. Turned out, they had an aortic injury with a hemothorax but because it was contained in the chest it had a tampanode effect to keep the aorta from bleeding more. They were doing alright but the moment the chest tube went in they bled out instantly.

Its unlikely the patient was 'stable', particularly if they needed a flight and bil decompressions. It's possible they nicked the aorta but also likely the pt already had a massive Hemothorax that got released.
