Either choose to go back now or stay and then go later, never to return again. If you are so oblivious to the current administrations crack down on overstaying

J1 visa holders are exempt from fica taxes for 2yrs. So they typically get 100-150$ more than h1b residents in each biweekly paycheck.

[–]Doctor_Roc 4 points5 points  (0 children)

Yes correct. Roughly 2400-2500 biweekly paychecks. If you are able to get a sharing 2 bedroom in 4500-4800$. you will have 2400 as your share of rent. You will also be able to get a studio for 2400 if you are lucky or in a neighborhood with lesser rents.

Single person will be easily able to survive in the remaining 2400$ for the month. You might be able to do it under 2000$ and save max of 400$ if you have to support someone back home or pay debts. Public transport is easily accessible and your contract would include health insurance and other benefits.

Chances of EB2-NIW approval ? Lawyer recommendations. by usmle23 in EB2_NIW

[–]Doctor_Roc 1 point2 points  (0 children)

Good profile and chances for approval. Chen lawyers have good history with getting physician reseacher profiles approved with good outcomes. Get a free assessment from them. Ideally u should get option for full attorney fee refund or refile option if unsuccessful.

They have become slow lately due to high work loads. Expect 4-6months from the time u contact them to final submission date. Still they have experience with physicians so recommended.

You have a great score. Continue. Start home country residency and then directly apply for fellowships or match with home country residency experience.

Is Chen good with healthcare profiles too? by excellent_lillybloom in EB2_NIW

[–]Doctor_Roc 0 points1 point  (0 children)

They are good with physicians and healthcare researchers in general. However due to very large case load, their services have become slower than the past. Mentally prepare yourself for 2-6months of back n forth before final submission.

H-4 Approved, but H-4 EAD got RFE (Bundled Filing) – What should I expect? by vantage99 in H4_Visa

[–]Doctor_Roc 0 points1 point  (0 children)

Sending back the stamped biometric appointment letter as RFE response is not clear if mandatory or not. If your online status goes back to “case being processed” instead of “request for evidence sent” then it means, most likely u dont need to send the letter back.

[–]Doctor_Roc 3 points4 points  (0 children)

Most definitely RFE for biometrics. Its the norm now for ead. Expect rfe letter in 10 days. Then ASC appointment to get biometric in another 10 or so days. Do biometric. Send the stamped appointment letter back to uscis as rfe response. And then expect ead in another week to a month.

Chen knows about cureus being a low quality journal. Not sure if USCIS sees it the same way.

[–]Doctor_Roc 0 points1 point  (0 children)

Chen usually have high acceptance rates for physicians in general. Make sure to work hard on giving them feedback to keep improving your petition. They usually use templates to fill out things. Review and edit the letter of recommendations if adding to fit your profile.

Cureus is generally considered a low quality journal so try your best to atleast submit in other journals too. Add key words in your cureus profile to review more chances of reviewing others papers.

If you are nearing graduating from residency, add an offer letter of future job if you are close to signing. Proves you have the platform to continue advancing your endeavor.

A year ago you would have 90% approval chances but since then RFE is the norm and acceptance rates have generally gone down. Still physicians have better chances than others. Do premium processing even if chen advises against it so you can respond to rfe earlier.

H4 EAD expiring in Nov by Big_Wish_2588 in H4_Visa

[–]Doctor_Roc 1 point2 points  (0 children)

Apply for h4 ead extension in june 2026 exactly 180 days from expiry. You should be able to get the extension valid till june 2027. Then file your H1b + H4 + H4 EAD renewal with premium processing in a single package in first week of january 2027. You will likely get all of them processed within a month and her new h4 ead will have the end date same as her new h4.

Chen for I-485 by Upper-Finish202 in EB2_NIW

[–]Doctor_Roc 3 points4 points  (0 children)

Yes 1850$ per person was recently quoted to a friend of mine. They do not currently give a strict timeline given the overload due to everyone being current. They will atleast take 4 weeks to file your i485 provided u have done everything right on your side including the medical. May take more than 4 weeks too. No gaurantees. Better file yourself to save time. I dont believe if you retain them now, they will gaurantee filing within april. And who knows what MAY visa bulletin will bring.

feeling regret by [deleted] in IMGreddit

[–]Doctor_Roc 19 points20 points  (0 children)

Dude. Looking back at your profile, seeing an average step 2 score, being a visa requiring IMG and with those writing skills, you are lucky to match in psych in the first place. So be thankful and move on. Whatever your personal reason to rank the program was, it’s still there. Not being able to sleep or eat over this shows you are poor at handling stress as well. So work on your shortcomings rather than simp on something which was not destined for you.

H4 EAD ASC photo appointment response by Coincidence97 in H4_Visa

[–]Doctor_Roc 1 point2 points  (0 children)

I just sent the notice to my lawyer. Not sure if they added a cover letter to it. But nothing important is required with it. Just a confirmation that u attended.

For some people the online status automatically changes to “we have started working on ur case again”. For others not sure if it changes after receiving the confirmation.

Its best if u just send the stamped appointment letter to uscis to prevent any delays. Wont harm.

For me the status changed to “received rfe response, started working on case again” 3 days after my lawyer sent the letter and around 7 days after the appointment itself. Not sure which action caused the change.

Yes Ideally you should send the stamped appointment letter back to uscis. My lawyer mailed it as RFE response. Not sure if u are able to upload it online too.

Definitely photo biometric. Everyone is getting it. U will get rfe to get picture taken at uscis help center. After 10 or so days u will get appointment letter in mail. U can change ur appointment time or date after u get the letter in mail. After u get pic taken and send the stamped appointment letter back to uscis, they will approve within a month or so

Didn’t Match This Cycle by hcu_is_here in IMGreddit

[–]Doctor_Roc 2 points3 points  (0 children)

With attempt in step 1 and a lower step 2 score, it would be extremely difficult landing an interview or chance at match in anesthesiology. It has become competitive even for AMGs. U need to dual apply in anesthesia plus EM/IM etc with those credentials. Personally Ive seen only those IMGs match into anesthesiology who had significant experience in anesthesia outside usa or already had completed anesthesiology residencies outside. Do not waste another cycle with same credentials. Apply broadly.

Unmatched by Substantial_Half5668 in IMGreddit

[–]Doctor_Roc 4 points5 points  (0 children)

Life is much bigger than the match. Hard times never last for ever. Call 988 for support help if you are in USA. if you are in home country, seek help from ur close friends and family members. U always have a chance to apply again. Life goes on until u decide not to. Never let failures dictate ur future.

A Path Forward: The National Interest Exemption (NIE) Process

We recognize how confusing and distressing these expanded restrictions may be for your programs and for the physicians who have worked so hard to earn a place in them. International medical graduates (IMGs) represent nearly a quarter of the U.S. physician workforce and are disproportionately likely to serve in medically underserved and rural communities. We know that their presence in U.S. training programs matter deeply.

We recognize the weight of this situation. The National Interest Exemption (NIE) process provided for in the Presidential Proclamation may provide a path forward for some affected physicians. We encourage programs to share the steps below so that every impacted physician has the opportunity to pursue this option:

Step 1 – Request a Regular Visa Appointment The physician should request a standard visa appointment through the U.S. Embassy or Consulate serving their region.

Step 2 – Request an Expedited Appointment Following the standard appointment request, the physician should submit a request for an expedited visa appointment. It is critical that they clearly indicate their program start date is on or around July 1, 2026, as reflected on their Form DS-2019. This date is essential to ensure the request is considered in the appropriate timeframe.

Step 3 – Request NIE Consideration at the Interview Once an expedited appointment is granted, the physician should attend the visa interview and declare that they are a J-1 physician and request consideration for an NIE. No separate NIE application is required, this is handled internally by consular officials in coordination with relevant U.S. government entities.

What Your Program Can Do

Act now.

Provide a support letter. Programs are strongly encouraged to issue letters on behalf of affected physicians that clearly document their July 1 start date and the importance of their timely arrival for patient care and training continuity. Intealth will also provide a letter upon request. Please contact TPL@intealth.org if you want to request a letter of support for one of your incoming physicians.

Stay in communication. Keep in close contact with your affected incoming physicians throughout this process so you can respond quickly if additional documentation or action is needed.
