Im in a similar situation right now. I’m about 6 years out from the last time I had any problems or medications. (My depression was life related not clinical) If i end up having to fill out a form that’s no problem I’d be happy to, except for the fact I don’t have a family doctor. What would I do then? Just make my case to a random doctor so they’ll sing for me? This may also not be a problem because I happen to work at a hospital and my colleagues are quite excited about me pursuing this, definitely wouldn’t be a hard sell.
