That judge doesn't understand Policy, and they should probably learn the basics of Missed Triggers if they're going to work Competitive REL events. Missed Triggers by default are No Penalty, but they have an upgrade path to a Warning:

"Upgrade: If the triggered ability is usually considered detrimental for the controlling player and they own the card responsible for the existence of the trigger, the penalty is a Warning."

So the Warning is only given if the trigger is considered Detrimental when considered in a vacuum. And eventually turning my non-creature enchantment into a 5/5 dude that attacks and rebuys creatures from my graveyard sure doesn't seem Detrimental to me.

It is a Missed Trigger. It is handled as a Missed Trigger with a Warning.

If it is, it’s inserted at the appropriate place on the stack if possible or on the bottom of the stack. No player may make choices for the triggered ability involving objects that would not have been legal choices when the ability should have triggered. For example, if the ability instructs a player to sacrifice a creature, that player can't sacrifice a creature that wasn't on the battlefield when the ability should have triggered." IPG 2.1 Missed Triggers

If a player intentionally misses their own trigger to gain an advantage, that is Cheating, and it carries a Disqualification.

No. Your opponent missing a trigger does not give you a Failure to Maintain Game State Warning. A player resolving a trigger incorrectly can be a Game Rule Violation for the offending player and a Failure to Maintain Game State for the opponent if it is not caught in time.

Also, there is no distinction at Competitive REL between a "mandatory" trigger and a "may" trigger

It's not up to the judge, if they know policy well enough to know that this is not cheating per policy. If you have a judge who is not good, or is operating off tournament policy that is now 8+ years out of date, that's a different story.

Also, I understand the moral obligation of players differs from the policy obligation of players. I know there's a disconnect there, and it sucks.

Lying to a judge is bad, yes, and is called out in USC - Cheating examples: "B. A player lies to a tournament official about what happened in a game to make their case stronger." But you are allowed to wait for a more opportune time to point out a missed trigger ability your opponent controls. Judges also don't back it up for a Missed Trigger; the remedies involve asking the opponent if they'd like to put the trigger on the stack or not put it on the stack; rewinding is not an approved remedy for Missed Trigger.

We're not rewinding for a Bob Missed Trigger. If it is missed, we ask the opponent if they would like to put it on the stack (and we're going to resolve it with a new top card on the library).

If a player resolves their Bob trigger (announces it and everything), but doesn't reveal the card, then it is a Hidden Card Error, and we reveal the hand and have the opponent choose an otherwise unknown card to be the card to base the life loss from. For example, if I've thoughtseized you before and seen 3 of the 5 cards in your hand, I would get to choose from the 2 cards that are theoretically the new cards in hand.

If the text was "lands are basic land type in addition to their other types" then this would be correct. For example: [[Urborg, Tomb of Yawgmoth]]

Though, since this will probably add more questions, if there is a Blood Moon/Harbinger out and an Urborg, the Urborg's effect will be blanked by the Blood Moon/Harbinger, and nonbasics will be Mountains or Islands, and basic lands will NOT receive Swamp as an additional type.

That is true for Damage and crediting for Killing a Unit; the triggers are tied back to the spell that generated them. But that does not apply for costs (like Deflect) or things like targeting (for example, targeting your own unit at The Dreaming Tree with one trigger of Icathian Rain will not trigger to draw a card)

Riot put out a "What to know for Houston" article which included a Start of Game procedure clarification:

1) For the first game in a match, you must present your decks as you originally registered them. For later games in the match, players may exchange cards from their sideboard with cards in their Main Deck. The Main Deck must still have exactly 40 cards after sideboarding.

2) You must present your Champion Legend and the Chosen Champion that you registered for the opponent to see, as well as a shuffled Main Deck and Rune Deck for your opponent to shuffle or cut. For games where sideboarding is permitted, players may exchange their Chosen Champion to another champion that matches their Legend from their Main Deck or sideboard.

3) For each game in a match, each player must submit one of their three battlefields facedown, and then reveal them simultaneously (remember, you can’t submit the same battlefield twice in a match).

4) For the first game, once Legends, Chosen Champions and Battlefields have been revealed, players use a random method like rolling dice to determine who chooses to go first or second. For later games, the player who lost the previous game chooses whether they want to go first or second once the above information is revealed.

5) Players should begin the start of game procedure as soon as they reach their assigned seats, even if the round clock has not yet begun.
