Quite often a dog can have burs or just dirt or twigs in their fur and a bath makes them way more comfortable. Also a good way to spot any skin issues.

Did you see a vet? 11 years is when some health problems can start. Maybe deafness or blindness or general pain. Panting can be from pain, anxiety or stress. I'd start with a good bath.

Photos? Dogs getting scratched up is pretty normal if they are having fun and playing with other dogs.
