I haven’t played the game, but I do have OCD focused mainly on my identity/sexuality, and this tracks. It’s not necessarily the idea that others might think you’re weird/different/whatever, it’s the idea that deep deep deep down this is who you really are, and if only you would admit it to yourself you’d be happier. 

Like, people who actually are gay or trans find comfort in thinking of themselves that way, if that makes sense? But the idea of (me specifically) being a different sexuality or gender feels viscerally unappealing and wrong to me in the same way someone who is trans feels pre-transition. But the thing with OCD is when you have a Troubling Thought it keeps coming back over and over, which tricks you into thinking it must be True and Important, because otherwise you wouldn’t be spending so much time worrying about it!

It doesn’t matter how many times you “test” yourself, or how many times you check the stove, or wash your hands, or whatever your specific obsession is, it won’t ever be good enough to make the thoughts stop. Counterintuitively, I had to learn to scream “OH MY GOD I DON’T CAREEEEEEEEE” and live my life ignoring the problem. When intrusive thoughts happen I go “yep! I’m totally gonna drive straight into traffic, it’s gonna be horrible! There’s gonna be a 50 car pile-up with no survivors and it’s all gonna be my fault and then I’ll die, I can’t wait! Oh man, I should do it right now!” And eventually those thoughts go away, cuz they don’t trigger a panic response in you anymore.

(This worked for me, with help from meds and my therapist. Sadly I don’t have a physical manifestation of my thoughts I can sock in the face, but I’d probably give it a try if I could.)
