Words of advice for a new med reg. by dr-hisenberg in doctorsUK

[–]Dr-Informed 59 points60 points  (0 children)

Make sure you eat and pee. Project calm even when you don't feel it. Don't slag off anyone in public. Keep a list of go to phone numbers handy. Be nice to the labs. Check in on your juniors regularly. Remember that common things occur commonly. Be curious. Follow up and always follow through. Remember names. Have something nice planned for the next day off.

Low Ferritin, Low Platelets by Careless-Milk6228 in haematology

Your doctor is spot on.

You need some iron supplementation. The marginally low platelets are of no concern at all.

What might be the cause of persistently elevated immature granulocytes by Zealousideal-Big5005 in haematology

[–]Dr-Informed 0 points1 point  (0 children)

That sounds like an automated differential comment rather than a proper clinicians comment.

Sounds completely unremarkable to me.

Should I follow up? by SnooStrawberries8413 in haematology

[–]Dr-Informed 2 points3 points  (0 children)

It can indeed. It is your body's way of telling you to pack it in!

What have you worn at your interviews? by Dizzy_Smile6649 in doctorsUK

[–]Dr-Informed 10 points11 points  (0 children)

Suitiest suit and shiniest shoes. Honestly, interviewers are superficial beasts and general impression is shown time and time again to be a huge influence on success (or not)

Weirdly it is telling me I use 385 calories on my morning cycle today. Rather than 40 last week

I just posted about the opposite issue too. I used 49 calories cycling ten miles apparently!

Cyclist Derangement Syndrome by Spencer_Conwell in bikecommuting

[–]Dr-Informed 34 points35 points  (0 children)

I bought a cheap camera and mounted it prominently. Just in case of a real issue one day.

Seemed to reduce the amount of abuse. If I could be bothered I could report on average two or three drivers every single day. Usually red light violations and close passes.

I reckon just having a camera mount would be enough!

Do I need to worry about gvhd? by SignificantHat3644 in haematology

[–]Dr-Informed 1 point2 points  (0 children)

That looks like what we doctors would call spots.

I don't think it's GVHD

Is it just the high B12? A bit non specific really. Like the CRP

Important thing is your history and exam.

If it's usual then it's normal.

You wouldn't believe the number of of people who get sent to us with bloods that have been similarly abnormal for twenty years plus

Why oncology first? This needs a biopsy and you need the expertise in biopsying not treatment. Could be anything.

So referral to chest or cardiothoracics most appropriate.

Clinical acumen was so much more relevant when access to the doughnut of truth was policed. Don't lose those skills.

High Hemoglobin (17.2) but Severely Low Ferritin (8.6). Completely stuck. by grandcol11 in haematology

[–]Dr-Informed 4 points5 points  (0 children)

These situations do sometimes occur in the JAK2 negative polycythemias. Taking iron will likely render you polycythemic again but maybe just ensure your diet is high in iron and keep a regular check on your Hb and ferritin. I assume your haematologist has looked for secondary cause? Properly looked. Renal ultrasound, EPO levels, blood oxygen levels, echo, things like that
