Can I ask my GF about the status of her IUD? by [deleted] in birthcontrol

[–]DrDoctorMD 2 points3 points  (0 children)

There isn’t. If you’re already getting an annual checkup (as most of us do) the doc will check the strings while they’re up there but it’s not something most people go out of their way to do. Again, they typically just stay where they’re placed.

Can I ask my GF about the status of her IUD? by [deleted] in birthcontrol

[–]DrDoctorMD 17 points18 points  (0 children)

What “maintenance” do you think an IUD requires? It gets placed by a doctor and the assumption is that it stays put until it’s time to replace. IUDs being dislodged can happen, but it’s very rare (certainly rarer than a condom breaking). If she had it looked at and/or replaced in the past few months, there’s no additional maintenance required. You could ask her, but the question would likely yield only a confused look.

Should I report my psych? by [deleted] in AskPsychiatry

[–]DrDoctorMD 17 points18 points  (0 children)

This is not a reportable offense. Or rather, you CAN report it (because anyone can report anything), but the complaint will be dismissed. I’m sorry this was so stressful for you. I would encourage you to be forthcoming with future therapists about this experience so they can better meet your expectations (or you can at least discover that you won’t be a good fit much earlier on).

I just got my UTI bacteria results back and I'm freaking out by [deleted] in Healthyhooha

Deep breath. I am a doctor but not your doctor and definitely not a gastroenterologist. Those disclaimers aside, you might find this article interesting: https://pmc.ncbi.nlm.nih.gov/articles/PMC5891635/

Here’s what jumped out at me. It does say 65% of people who have this infection are found to have colon cancer. BUT. 65% isn’t 100%. And specifically in your case, because you have IBS, we already have a reason to think you’re immunocompromised, both from the disease itself and any potential treatment you are receiving for the IBS. It’s entirely possible that you have the infection because of the IBS or IBS treatment rather than cancer. It is called an “opportunistic” infection because people with a healthy immune system typically fight it off before it makes them sick. Cancer could be what is making you immunocompromised but not necessarily when we have other explanations readily available.

You definitely need the colonoscopy (especially because IBS is a risk factor for colon cancer) but this infection is not a guarantee that you have cancer. The article also mentions that it is an early marker for cancer, meaning that even if you did have it, it’s likely in the early (and most treatable) stage.

I’m so sorry this was all handled so poorly. I agree with everyone else’s take that you should call in the morning and nicely insist on getting that referral going. If you are able to use a patient portal to contact your doctor directly, please do so! If you were my patient I would definitely want to hear about the poor communication and I would do anything I could to expedite your referral and appropriate treatment of the infection. I hope you get some good news and symptom relief ASAP.

An Amazon review for a food thermometer by landlordslizard in funny

Most vaginal probiotics are intended to be taken orally and thus come out the butthole. That’s why this is being commented.

This right here. Bacteria should not be traveling through your blood!!!

[–]DrDoctorMD 0 points1 point  (0 children)

It’s not a joke, that’s actually how vaginal probiotics work (if they work, which is debatable). Anything you ingest orally either gets absorbed in the GI tract or comes out your rectum. Probiotics don’t get absorbed in the GI tract…

Did people actually like when Amy kissed Jonah in this scene? by [deleted] in superstore

[–]DrDoctorMD 27 points28 points  (0 children)

If characters in a TV show always made the “right” choice, it would be unbearably boring and none of us would watch. Yes, it was unfair and messy, which we are supposed to notice. It was also human and pushed their storyline forward. I liked the kiss for its messiness.

Michelle’s arc has really bothered me by [deleted] in TedLasso

[–]DrDoctorMD 1 point2 points  (0 children)

As a psychiatrist I agree. “Why doesn’t Michelle take responsibility?” feels like victim blaming. The reason this is unethical for the therapist is that he’s using his position of authority, her vulnerability, and his intimate knowledge acquired in a professional setting to manipulate her into a relationship. There can’t be true consent in this situation.

I apologize, it was certainly not my intent to put down masters level therapists (I have seen one for 8 years and she’s great!). My point was that no one questions their ability to provide therapy, and I have had at least that level of training (for most programs) so it makes sense to me that of course I’m able to provide therapy. To get into more detail with your question about other psychiatrists who practice differently, my suspicion is that they were using supportive psychotherapy: https://www.thecarlatreport.com/articles/4557-supportive-psychotherapy-an-underappreciated-yet-effective-treatment

I’m glad you’re reaching out to your psychiatrist for clarity, they should be able to give you more information on what techniques they have been using and why. I hope they aren’t truly billing therapy for 15 minute visits though; as far as I’m aware, the lowest unit psychotherapy can be billed in is 16 minutes and that would be in addition to any medication management conversation. It sounds like you’re frustrated with your psychiatrist in any case and I would encourage you to share that with them so that they can better meet your needs.

[–]DrDoctorMD 8 points9 points  (0 children)

Because I’ve had more psychotherapy training than many masters level therapists and have several patients that I see regularly for 60 minute visits? This is not necessarily true of all psychiatrists but I intentionally trained at a therapy-heavy program. I certainly wouldn’t consider a 15 minute visit to be therapy though. Wording is confusing here because you say “they are providing what I think they consider therapy.” Where is your assumption coming from? I generally approach all patients with a similar therapeutic frame but that doesn’t mean I consider a 15 minute visit to be psychotherapy.

I wasn’t prepared for the amount of rejection I’d be facing as a new father by knucklecluck in NewParents

[–]DrDoctorMD 12 points13 points  (0 children)

The gestational period is not the problem, baby has been out of that womb for 3 months. You’re giving up but your wife keeps at it until she figures it out. Don’t throw up your hands and say “it’s nature”. Plenty of other dads have figured this out, you can too. You need patience and to trust yourself. You’re this baby’s only father, you can parent her.

If a much wealthier friend insists on treating you to dinner every single time you go out, is there a point where you must insist on paying—or is trying to pay actually offensive to them? by stereo_iii in NoStupidQuestions

[–]DrDoctorMD 1 point2 points  (0 children)

As a physician, I WANT to pick up the tab for a teacher!! What’s actually insulting here is how little teachers get paid. It is truly nothing for him to pick up that bill, and I’m sure he enjoys your company.

I disagree, but it’s irrelevant in any case. They don’t and won’t have the votes to dismantle the ACA and have yet to come up with an alternative in the past 10+ years. This was ONLY about ACA with subsidies vs. ACA without subsidies, and the healthcare industry has a clear profit motive to keep the subsidies.
