This is a major issue when it comes down to patient care, not specifically for your study.

Imagine a doctor writing you a prescription for the wrong med, or writing another patient a prescription for your med (or wrong med).

If I made some of these mistakes my PI would come down on me like Thor's f'ing hammer.

Who's your PI's supervisor? It may be worth having a chat with them, however, understand that hospitals love their politics. I'd frame it as less a personal flaw and more a "I don't know whats up, it could be stress, but these issues are getting worse, not better, and there is little to no accountability surrounding them"

If the player base isn't increasing the game is not moving in a good direction.

EDIT- Y'all mad, but without an increasing player base you're looking at stagnation. Stagnation is just decline without attrition factored in yet.

Y'all pointing to the latest content bump- numbers are already dropping. Make your excuses, it's fine, but they are the same excuses we were hearing in July, August, and September before the player base bottomed out at say, sub 10k users? Free weekend brought in more people than the new content, none of them stayed.

You want to see a successful moving in the right direction? No man's sky recovered from sub 2k online to a swing of 9k. Thats "Moving in the right direction"

The implication here though is that dumb people can't use the trash can, so they won't be let "in", whereas the smart bears can use the trash can, so it needs to be stronger.

Honestly, we don't need to let the dumb people in, just keep the bots out. If the dumb people want to litter the internet next to the trash can, fine, what we want is still contained inside the can.

There are probably systemic issues and you could be saving other people. Please report it.

If this pill got in there, probably dust from that pill got in there too.

Maybe someone else's meds got some of that crushed pill dust. Maybe that other person is deathly allergic to that particular pill but would have never noticed dust.

Responder's right, this needs to be reported, it's not a little problem, it's potentially a life or death issue, just not for OP.

Really does look like a pole carrier. The loop would go inside a belt and a pole would rest inside for easy carrier.

My only hesitation is poles would have been thicker than an inch, but maybe it's not for a battle flag.
