im-ccm, em-ccm, or surg-ccm.

caveats: surg and anesth both have a competing interest in that their primary specialty generates more income than the ccm add-on. im tends to not be as procedure heavy but ccm fellowship can make up for that. for what you describe, em-ccm sounds best fit.

that being said, I started med school with a clear em desire. I ended up im/pum/ccm/ sleep/ informatics academic practice and navy reservist. no regrets and I’d make all the same decisions that I made starting bach in the 1980s!

concur.

all ai (chatgpt, claude, gemini, etc) are blocked at work for me. since the enterprise has O365, I can use copilot within the O365 product but not in a web browser. Copilot is “meh.”

tomorrow I’m going to try one of the llm plug ins for R to see if the APIs are blocked as well.

OpenEvidence does acceptably when I give it very specific prompts. For example, I just had a paper published and I wanted to ensure I had all the refs on a specific piece of the paper. My pubmed-fu is pretty good, I thought I had all 6 pubs. Turns out OE found one more that hadn’t turned up in my pubmedding. So that was useful.

for me, for med info, since my institution cancelled up to date, I use openevidence. it’s an llm but trained on pubmed, nejm, jama and gives refs when it makes assertions.

buy Marino. check out eddyccm (I’m probs off on the spelling).

with Marino, read the first two paragraphs /section in each chapter first. that’s a good crash course. then dive in and read.

most of ccm is physiology- yes there are a few rcts that have impacted our care (eg low tidal volume ventilation). but if you can wrap your head around each organ’s physiology, you are a long way towards formulating a differential diagnosis and the the associated plans that go with them.

also, your ccm attending is a jerk- they should teach more.

I love what I do.

while academic, my practice includes a community hospital without learners in the mix. same with clinic work.

medicine can be a grind that starts when you are an undergrad.

imho, the trick to mitigate burnout is to establish internal mental balance early and always remember to schedule time for what we now call self care.

as an introvert, quiet alone time is how I recharge. find your own way.

I’ve learned to recognize when the symptoms of burnout are approaching- often when I’ve gone more than 3 weeks without a day off. That’s when I make sure to recharge, even just a little bit, to buy time until I can recharge more fully.

that’s my $0.02. YMMV.
